A copy of this form shall be attached to each of TWO sets of plans, of <br />m inim um 11” x 17” size. Will the PV system be installed on a roof havin g only on e roofing layer with no <br />overlays? <br />3. Will the PV array be flush m oun ted to the existin g roof so that the plan e of the <br />m odules (pan els) are parallel to the plan e of the roof? <br />4. Will the PV system weigh m axim um 4 pounds per square fee t or less? <br />5. Will the PV system be installed where the m odules do not overhan g an y roof edges <br />(such as eaves, gabled en ds, ridges an d hips)? <br />6. Will the PV system be installed with a space of 2” m inim um to 10” m axim um betwee n <br />the un derside of m odule s and the surface of the roof? <br />7. Will the PV system be installed without usin g any ballast system or coun ter-weight <br />system ? <br />8. Will the anchors be installed with a m axim um horizontal anchor spacin g of 6 feet <br />an d is this m axim um horizontal spacin g shown on the plans? <br />9. Will the m in im um 5/16” lag screw s be in stalled with a m inim um of 2-1/2 inch <br />em bedm en t into roof rafters (with pre-drilled holes) and is this m inim um em bedm en t <br />shown on the plan s? <br />10.
