sign the certification section below. A copy of this fom shall be attached to each of TWO sets of plans, of

per Section 605.1 1 of the California Fire Code, and is this shown on the roof plan?

Are ALL the structural pages of the plans WET stamped and signed by a Califomia
licensed professional engineer? (including project speciflc site plan, PV layout,
anchorage spacing, anchorage details and manufacturer's PV support information.)
