Venting is prior to tank through normal plumbing. There is no additional venting
required for any Pro-Cast Produds Grease lnterceptor, Clarifier or SepticTank.

RE: Santa Ana EOS Fitness Fan Mounting
The MacroAir manual states that the fan is to be mounted with Unistrut on joists at eight feet on
center. We understand that this EOS fitness has joists at ten feet on center. The attached
calculation shows that in order to maintain equivalent bending moment in the Unistrut the fan
must be mounted outside of the middle 4.5 feet of the span.

I visited the subject site on April 15,2019 to observe the structural repairs
made to the fire damaged portion of the building and to determine if the
repairs were in general conformance with the structural repair plans
prepared by Hansen-Hokama Structural Engineers. Based on my site
observations the structural repairs are in general conformance with the
plans. The roof diaphragm nailing' the roof strapping, seismic wall ties
and addition of ledger bolts at the purlins were in general conformance
with the plans. Therefore, the roof may be covered with roofing material.

California Residential Code (CRC) Section R314.1. and R315.2 states in part that existing dwellings be
"retrofitted with smoke alarms and carbon monoxide alarms. CRC Section R314.3 and R314.3.3 define
the requ ired locations.

are still valid and should be properly implemented, where applicable.
The proposed building footing trenches can be excavated, and construction of the proposed

It is noted that the remedial removal and recompaction of the upper 3 ft. thick pre-existing
earth materials olthe building pad area and minimum 3 ft. beyond/outside of the perimeter of
the proposed building has been performed and the fill / backfill earth materials were
compacted to minimum ol 90% of the maximum density determined in accordance with the
ASTM Test Method D1557.

EQUIPMENT SPECIFICATIONS
All new and replacement food-related or utensil-related equipment shall be certified or classified for SANITATION by anAmerican National Standards lnstitute (ANSI) accredited certification program. Example of these accredited programsinclude NSF, UL, lntertek ETL, CSA etc.
All new and replacement electrical.appliances shall meet applicable Underwriters Laboratories (UL) standards forelectrical equipment as determined by an ANSI accredited certification program.

2. Sneeze guard protection must be provided where food is displayed or customer self-service foods are proposed.
Displays of unpackaged food shall be shielded so as to intercept a direct line between the customer,s mouth;assumed
to be between 54 and 60 inches above the floor, and the food being displayed.
HOOD CONSTRUCTION
lvlechanical exhaust ventilation shall be required at or above all cooking equipment such as ranges, griddles, ovens,deep fat fryers, barbecues, and rotisseries to effectively remove cookirig odois, smoke, steam, lrease, and vapors.Hoods and mechanical ventilation must comply with all applicable requi-rements as set forth in tf,e current edition of theUniform Mechanical Code, as adopted by the local building and fire authorities.

Effective December 15,2004, the City of Santa Ana adopted Ordinance Number NS 26-70 amending Chapter
39 of the Santa Ana l/unicipal Code adding fats, oils and grease (FOG) conkol regulations applicable to Food
Service Establishments (FSES). The ordinance requires all new and remodeled FSES to install and maintain
grease interceptors. FSEs without grease interceptors and that meet certain criteria are hereby issued a
conditional waiver under the terms of this Notice and Conditional Waiver.
FSEs will be routinely inspected to evaluate the effectiveness of their grease control measures. Existing FSES
that discharge FOG and are out of compliance with the Ordinance provisions will be required to install grease
interceptors. All FSES must implement Kitchen Best Management Practices (BMPs) to remove FOG from the
waste stream entering the sewer system.
Program Rules and Regulations
The City has established Rules and Regulations for the FOG Program. Refer to the City website at

Conditional Waiver
A Conditional Waiver to the requirement to install a grease interceptor is hereby granted. This waiver may be
revoked at any time upon a determination of one or more of the following:
Quantity of fats, oils and grease (FOG) discharge as measured or as indicated by the size of the food
service establishment (FSE) based on seating capacity, number of meals served, menu, water usage,
amount of on-site consumption of prepared food and other conditions have changed since the
granting of said waiver such that the FSE is contributing to FOG discharges.
ldentification of the FSE as a significant contributor of FOG into the Sewer System, which is based on
inspection or sampling of the FOG discharged from the FSE'S sewer lateral to the Sewer System.
Adequacy of implementation and compliance with Kitchen Best Management Practices (BlvlPs).
Change in sewer size, grade, and condition based on visual information.
Changes in operations that significantly affect FOG discharge.
Failure to comply with any of the specific requirements set forth below in this Conditional Waiver.
Any other condition deemed reasonably related to the generation of FOG discharges by the FOG
Control Program Manager.

expansion of dining area, and or change in type of operations or use, are being performed at this time. The
developer/owner has been notified that any improvements, that include what is mentioned above, will result in

Regulations, any applicable pretreatment regulations, standards or requirements under local, State, and

Effective August 1, 2017, the City of Santa Ana adopted Ordinance Number NS 2921 afiending Chapter 39
of the Santa Ana Municipal Code adding fats, oils and grease (FOG) control regulations applicable to Food
Service Establishments (FSES). The ordinance requires all new and remodeled FSES to install and maintain
grease interceptors. FSES without grease interceptors and that meet certain crileria are hereby issued a
conditional waiver under the terms of this Notice and Conditional Waiver.

This letter to certify that the swimming pool at rear of property had filling material
generally consisted of medium light brown sand with some sill (have expansion
index less than 20) have been compacted to a minimum of 90olo every 1 feet lift of
maximum density and is consider geotechnical acceptable.
If permitted by the City, a structural objected such as a room addition can be constructed at or
near the swimming pool back fill area in the future. Consultation with a qualified geotechnical
engineer should be considered for provision of geotechnical recommendation for structural and
foundation design.

Per a 10:07 a.m. phone conversation with Wesley Chu, the owner, he would like to proceed with epoxy setting of
12mm (.472"1all thread bolts despite approved plans calling lor 5/8". Mr. Chu understands that this report will be
designated as non-compliant pending city approval. ln addition, pull test performed on 4/30/19 was lor 5/E" epoxied
all thread bolts only.
Visual inspection for drilling concrete, cleaning of holes, and epoxy setting of 12mm all thread bolts to minimum
embedment depths ol 3 1/8" per details 1.'l & '1.2/S1'15 for boulder wall frames B & C, inclusive of support beams
C01-C35 (26, 2 bolt patterns & 9, 4 bolt patterns). Correct bolt pattern locations and sequence per S 104.
Hilti HIT 200 epoxy (exp. 09/19) was utilized lor 4 bolt pattern beam legs and Red Head G5+ epoxy (exp. 01/20) was
utilized for 2 bolt pattern beam legs per same details as above.

Subject: STRUCTURAL OBSERVATION
A structural observation was performed for the above job on the morning of Wednesday June
10, 2019 by James Kramer, SE of our office. Structurally observed was; seismic wall
anchorage and roof framing strengthening for the north third of the project (grids 28 to 42, Alo
R).
With the exception of the items noted below, all observed construction appeared to be in
general conformance with the construction documents.
At the time of the observation, 100% of the wall anchorage and 100% of roof strengthening in
the area reviewed had been completed.
