Observed the preparation and epoxy installation of the following
#5 rebar dowels top and bottom at 24 o/c with a minimum 6" embedment per detail on D1 for new
to existing footing connections, located rear of existing structure foundation.
All holes were drilled 3/4' diameter and cleaned using compressed air and brushing. Simpson SET
XP was used with an expiration dale of 0'1127122. All work observed is in accordance with approved
plans, specifications.'
