At your request, Ladera Engineering Group has reviewed the referenced documentation to determine the structural integrity of the
existing monopole due to the carrier’s proposed equipment configuration mounted on the tower. The following information was
provided as documentation to assist Ladera Engineering Group with the structural verification:

The proposed scope of work includes adding (4) CBNG antennas and (1) Raycap 3315 to the carrier’s existing antenna mounts
at 39.0 ft above ground level.
In reviewing the original tower design loading by Vector Engineers dated September 18, 2015, Verizon’s final loading configuration
on this project has less wind EPA and less total weight then original loading used for the tower design. As such, Ladera
Engineering Group considers the existing monopole tower and its components to have enough capacity to adequately
carry the carrier’s final loading configuration . The assessment of the carrier’s structural mount is not included in the scope of
this project and is to be certified by others.
All referenced supporting documents have been obtained from the client and are assumed to be accurate and applicable to the
subject site. For the purposes of this assessment, it is assumed that an overall structure condition as erected and all members and
connections to be free of corrosion and or structural defects. The contractor is responsible for means and methods of construction
and shall notify Ladera Engineering Group immediately if any condition differs from what is listed above.
