Plan Submittal Criteria
COMMERCIAL projects, MULTIFAMILY RESIDENTIAL projects
and RESIDENTIAL TRACT developments
· Fill in the project/business address and provide a brief description of the scope of work and type of business operation that will take place.
· Answer questions 1 through 10, read and initial items 11 and 12, then complete and sign the certification section.
· If you answer: - “YES” to any part of questions 1 through 10, submit the type of plan indicated in italics to OCFA.
· In some cases, other plan types not indicated herein may also be necessary depending on specific conditions or operations.
· Visit www.ocfa.org for submittal information and locations. If you need assistance in filling out this form or have questions regarding

Project Scope/Business Description
1. Construction of a new building, a new story, or increase the footprint of an existing building? Changes to roadways,
curbs, or drive aisles? Addition, relocation, or modification of fire hydrants or fences/gates? Construction within
300 feet of an active or proposed oil well? Fire Master Plan (PR145)
2. Property is adjacent to a wildland area or non-irrigated native vegetation?
Fire Master Plan (PR145); a Fuel Modification Plan may also be required. (PR120, PR124)
3. Located in or < 100’ from a Division of Oil, Gas, and Geothermal Resources (DOGGR) field boundary, < 300’ from
an oil/gas seep, or < 1000’ from a landfill? Methane Work Plan. (PR170)
4. Installation/modification/repair of underground piping, backflow preventers, or fire department connections serving
private fire hydrant/sprinkler/standpipe systems? Underground Plan. (PR470, PR475)
5. Drinking/dining/recreation/meetings/training/religious functions or other gatherings in a room > 750 sq.ft. (> 1,000
sq.ft. for training/adulteducation) or > 49 people? Healthcare/outpatient services for > 5 people who may be unable
to immediately evacuate without assistance? Education for children (academic tutoring for ages 5+ is exempt unless
classified as an E occupancy by the Building Official)? Adult/child daycare? 24-hour care/supervision? Incarceration
or restraint? Hotel/apartment or residential facility with 3+ units and 3+ stories (3-story townhouses/rowhouses
where an independent direct exit to grade is provided for dwelling are exempt)? Congregate housing/dormitories
with 17+ people? High-rise structure (55+ feet to highest occupied floor level)? Architectural Plan (PR200-PR285)
6. Installation/modification of locks delaying or preventing occupants from leaving a space or requiring use of a card,
button, or similar action to open a door in the direction of exit travel? Architectural, Sprinkler, and/or Alarm Plan
depending on the occupancy and type of device installed (PR200-PR280, PR420-PR425, PR500-PR520)
7. Installation/modification/use of spray booths; dust collection; dry cleaning; industrial ovens/drying equipment;
industrial/commercial refrigeration systems; compressed gasses; tanks for cryogenic or flammable/combustible
liquids; vapor recovery; smoke control; battery back-up/charging systems (> 50 gal. electrolyte, > 1,000 lb. lithium
ion); welding/brazing/soldering, open flame torches, cutting/grinding; or other similar operations?

8. Storage/use/research with flammable/combustible liquids or other chemicals? Motor vehicle/aircraft
maintenance/repair? Cabinetry/woodworking/finishing facility? Chem Class & floor plan (full architectural plan if
H occupancy); Special Equipment Plans may be necessary. (PR315-PR360, PR232-PR240)
9. Storage or merchandizing areas in excess of 500 sq. ft. where items are located higher than 12’ (6’ for high-hazard
commodities, plastic, rubber, foam, etc.)? High-piled Storage Plan (PR330)
10. Cooking under a Type I commercial hood; installation or modification of a fire extinguishing system located in a
commercial cooking hood? Hood & Duct Extinguishing System, not just the hood mechanical plan. (PR335)

6. UNLESS NOTED OTHERWISE, THE WORK SHALL INCLUDE FURNISHING MATERIALS, EQUIPMENT,
APPURTENANCES, AND LABOR NECESSARY TO COMPLETE ALL INSTALLATIONS AS INDICATED
ON THE DRAWINGS.
7. PLANS ARE NOT TO BE SCALED. THESE PLANS ARE INTENDED TO BE A DIAGRAMMATIC
OUTLINE ONLY UNLESS OTHERWISE NOTED. DIMENSIONS SHOWN ARE TO FINISH SURFACES
UNLESS OTHERWISE NOTED. SPACING BETWEEN EQUIPMENT IS THE MINIMUM REQUIRED
CLEARANCE. THEREFORE, IT IS CRITICAL TO FIELD VERIFY DIMENSIONS, SHOULD THERE BE
ANY QUESTIONS REGARDING THE CONTRACT DOCUMENTS, THE SUBCONTRACTOR SHALL BE
RESPONSIBLE FOR OBTAINING A CLARIFICATION FROM THE ARCHITECT/ENGINEER PRIOR TO
PROCEEDING WITH THE WORK. DETAILS ARE INTENDED TO SHOWN DESIGN INTENT.
MODIFICATIONS MAY BE REQUIRED TO SUIT JOB DIMENSIONS OR CONDITIONS AND SUCH
MODIFICATIONS SHALL BE INCLUDED AS PART OF WORK AND PREPARED BY THE
ARCHITECT/ENGINEER PRIOR TO PROCEEDING WITH WORK.
8. THE SUBCONTRACTOR SHALL INSTALL ALL EQUIPMENT AND MATERIALS IN ACCORDANCE WITH
MANUFACTURER'S RECOMMENDATIONS UNLESS SPECIFICALLY STATED OTHERWISE.
9. IF THE SPECIFIED EQUIPMENT CANNOT BE INSTALLED AS SHOWN ON THESE DRAWINGS. THE
SUBCONTRACTOR SHALL PROPOSE AN ALTERNATIVE SPACE FOR APPROVAL BY THE
ARCHITECT/ENGINEER PRIOR TO PROCEEDING.
10. GENERAL CONTRACTOR SHALL BE RESPONSIBLE FOR THE SAFETY OF WORK AREA, ADJACENT
AREAS AND BUILDING OCCUPANTS THAT ARE LIKELY TO BE AFFECTED BY THE WORK UNDER
THIS CONTRACT. WORK SHALL CONFORM TO ALL OSHA REQUIREMENTS AND THE LOCAL
JURISDICTION.
11. GENERAL CONTRACTOR SHALL COORDINATE WORK AND SCHEDULE WORK ACTIVITIES WITH
OTHER DISCIPLINES.
12. ERECTION SHALL BE DONE IN A WORKMANLIKE MANNER BY COMPETENT EXPERIENCED
WORKMAN IN ACCORDANCE WITH APPLICABLE CODES AND THE BEST ACCEPTED PRACTICE.
ALL MEMBERS SHALL BE LAID PLUMB AND TRUE AS INDICATED ON THE DRAWINGS.
13. SEAL PENETRATIONS THROUGH FIRE RATED AREAS WITH UL LISTED MATERIALS APPROVED BY
LOCAL JURISDICTION. SUBCONTRACTOR SHALL KEEP AREA CLEAN, HAZARD FREE, AND
DISPOSE OF ALL DEBRIS.
14. WORK PREVIOUSLY COMPLETED IS REPRESENTED BY LIGHT SHADED LINES AND NOTES. THE
SCOPE OF WORK FOR THIS PROJECT IS REPRESENTED BY DARK SHADED LINES AND NOTES.
SUBCONTRACTOR SHALL NOTIFY THE GENERAL CONTRACTOR OF ANY EXISTING CONDITIONS
THAT DEVIATE FROM THE DRAWINGS PRIOR TO THE BEGINNING CONSTRUCTION.
15. SUBCONTRACTOR SHALL PROVIDE WRITTEN NOTICE TO THE CONSTRUCTION MANAGER 48
HOURS PRIOR TO COMMENCEMENT OF WORK.
16. THE SUBCONTRACTOR SHALL PROTECT EXISTING IMPROVEMENTS, PAVEMENTS, CURBS,
LANDSCAPING AND STRUCTURES. ANY DAMAGED PART SHALL BE REPAIRED AT
SUBCONTRACTOR'S EXPENSE TO THE SATISFACTION OF THE OWNER.
17. THE SUBCONTRACTOR SHALL CONTACT UTILITY LOCATING SERVICES PRIOR TO THE START OF
CONSTRUCTION.
18. GENERAL CONTRACTOR SHALL COORDINATE AND MAINTAIN ACCESS FOR ALL TRADES AND
SUBCONTRACTORS TO THE SITE AND/OR BUILDING.
19. THE GENERAL CONTRACTOR SHALL BE RESPONSIBLE FOR SECURITY OF THE SITE FOR THE
DURATION OF CONSTRUCTION UNTIL JOB COMPLETION.
20. THE GENERAL CONTRACTOR SHALL MAINTAIN IN GOOD CONDITION ONE COMPLETE SET OF
PLANS WITH ALL REVISIONS, ADDENDA, AND CHANGE ORDERS ON THE PREMISES AT ALL
TIMES.
21. THE GENERAL CONTRACTOR AND SUBCONTRACTOR SHALL PROVIDE PORTABLE FIRE
EXTINGUISHERS WITH A RATING OF NOT LESS THAN 2-A OT 2-A:10-B:C AND SHALL BE WITHIN 25
FEET OF TRAVEL DISTANCE TO ALL PORTIONS OF WHERE THE WORK IS BEING COMPLETED
DURING CONSTRUCTION.
22. ALL EXISTING ACTIVE SEWER, WATER, GAS, ELECTRIC, AND OTHER UTILITIES SHALL BE
PROTECTED AT ALL TIMES, AND WHERE REQUIRED FOR THE PROPER EXECUTION OF THE
WORK, SHALL BE RELOCATED AS DIRECTED BY THE ARCHITECT/ENGINEER. EXTREME CAUTION
SHOULD BE USED BY THE SUBCONTRACTOR WHEN EXCAVATING OR DRILLING PIERS AROUND
OR NEAR UTILITIES. SUBCONTRACTOR SHALL PROVIDE SAFETY TRAINING FOR THE WORKING
CREW. THIS SHALL INCLUDE BUT NOT BE LIMITED TO A) FALL PROTECTION, B) CONFINED
SPACE, C) ELECTRICAL SAFETY, D) TRENCHING & EXCAVATION.
23. ALL EXISTING INACTIVE SEWER, WATER, GAS, ELECTRIC, AND OTHER UTILITIES, WHICH
INTERFERE WITH THE EXECUTION OF THE WORK, SHALL BE REMOVED, CAPPED, PLUGGED OR
OTHERWISE DISCONNECTED AT POINTS WHICH WILL NOT INTERFERE WITH THE EXECUTION OF
THE WORK, AS DIRECTED BY THE RESPONSIBLE ARCHITECT/ENGINEER, AND SUBJECT TO THE
APPROVAL OF THE OWNER AND/OR LOCAL UTILITIES.
24. THE AREAS OF THE OWNER'S PROPERTY DISTURBED BY THE WORK AND NOT COVERED BY
THE TOWER, EQUIPMENT OR DRIVEWAY, SHALL BE GRADED TO A UNIFORM SLOPE, AND
STABILIZED TO PREVENT EROSION.
25. SUBCONTRACTOR SHALL MINIMIZE DISTURBANCE TO THE EXISTING SITE DURING

36. SUBCONTRACTORS SHALL BE RESPONSIBLE FOR OBTAINING ALL PERMITS AND INSPECTIONS

Ladera Engineering Group is pleased to submit this “Mount Analysis Report” to determine the structural integrity of AT&T Mobility’s antenna
mounting system with the proposed appurtenance and equipment addition on the abovementioned supporting tower structure. Analy sis of the
existing supporting tower structure is to be completed by others and therefore is not part of this analysis. Analysis of the antenna mounting
system as a tie-off point for fall protection or rigging is not part of this document.
The purpose of the analysis is to determine acceptability of the mount stress level. Based on our analysis we have determined the mount stress
level to be:
Pipe Mount Sufficient
This analysis utilizes an ultimate 3-second gust wind speed of 95 mph as required by the 2022 CBC. Applicable Standard references and
design criteria are listed in Section 2 - Analysis Criteria.

3.1) Analysis Method
RISA-3D (Version 22.0.1), a commercially available analysis software package, was used to create a three-dimensional model of the
antenna mounting system and calculate member stresses for various loading cases.
Load Calculator v1.5, a tool internally developed by Ladera Engineering Group, was used to calculate wind and seismic loading on all
appurtenances, dishes and mount members for various loading cases. Selected output from the analysis is included in Appendix B
“Software Input Calculations”.
3.2) Assumptions, Liability Waiver, and Limitations
1) The antenna mounting system was properly fabricated, installed and maintained in good condition in accordance with
its original design and manufacturer's specifications.
2) The configuration of antennas, mounts, and other appurtenances are as specified in Table 1 and the referenced
drawings.
3) All member connections are assumed to have been designed to meet or exceed the load carrying capacity of the
connected member unless otherwise specified in this report.
4) The analysis will be required to be revised if the existing conditions in the field differ from those shown in the above -
referenced documents or assumed in this analysis. No allowance was made for any damaged, missing, or rusted
members.
5) All bolted connections are pretensioned in accordance with Table 8.2 of the RCSC, 2014 Standard.
6) Steel grades have been assumed as follows, unless noted otherwise:

Connection Bolts ASTM A325
U-Bolts ASTM A307
This analysis may be affected if any assumptions are not valid or have been made in error. Ladera Engineering Group should
be notified to determine the effect on the structural integrity of the antenna mounting system.

3.3) Liability Waiver and Limitations
Our structural calculations are completed assuming all information provided to Ladera Engineering Group is accurate and applicable
to this site. For the purposes of calculations, we assume an overall structure condition as erected and all members and connections
to be free of corrosion and/or structural defects. The structure owner and/or contractor shall verify the structure’s condition prior to
installation of any proposed equipment. If actual conditions differ from those described in this report, Ladera Engineering Group
should be notified immediately to assess the impact on the results of this report.
Our evaluation is completed using industry standard methods and procedures. The structural results, conclusions and
recommendations contained in this report are proprietary and should not be used by others as their own. Ladera Engineering Group
is not responsible for decisions made by others that are or are not based on the stated assumptions and conclusions in this r eport.
This report is an evaluation of the mount structure only and does not determine the adequacy of the supporting structure, oth er
carrier mounts or cable mounting attachments. The analysis of these elements is outside the scope of this analysis, are assumed to
be adequate for the purpose of this report and to have been installed per their manufacturer requirements. This document is not for
construction purposes.

4.1) Recommendations
The mount has sufficient capacity to carry the proposed loading configuration. In order for the results of the analysis to be
considered valid, the following must be completed.
1. All bolts and hardware should be checked for tightness and condition prior to installing the proposed equipment.
2. Pursuant to ATT-CEM-24002, the proposed Ericsson AIR equipment in position 2 [from front of mount looking towards
building] requires a minimum mount pipe diameter of 2.875” OD. The pipe in this position, per sector, shall be (1) 2.5 STD
Pipe x 8’ long (CEQ# ANT.16008 or approved equal) with existing hardware.
No structural modifications are required at this time, provided that the above-listed changes are implemented.

Project: CLL03237 – Battery Cabinet Replacement
Site: 625 & 625 ¾ Grand Ave., Santa Ana, Orange County, CA, 92701
Subject: Opinion on Proposed Battery Cabinet Relocation/Replacement (Antennas/RRUs Excluded)
Scope of this Opinion
This letter addresses only the new battery work shown in the proposed construction drawings dated June 10,
2025, for project CLL03237. The scope includes removal of twelve (12) existing batteries from a
decommissioned cabinet and installation of one new battery cabinet with a fully loaded weight of 2,300 pounds.
All antennas, radios, and mounts are specifically excluded from this opinion.
Existing Basis & Prior Modeled Weight
The most recent structural analysis for this site included a battery cabinet modeled at approximately 3,250
pounds fully loaded. That configuration passed under the 2019 CBC / TIA-222-H design criteria and formed the
structural baseline for the equipment area.
Proposed Change (Weight Effect)
The new cabinet, at 2,300 pounds fully loaded, represents a reduction of roughly 950 pounds from the
previously analyzed cabinet weight. This results in lower gravity and seismic inertial demands on the
equipment platform and supporting structure compared to the prior design condition.
Code Basis for Current Work (Battery Cabinet Only)
The proposed cabinet and anchorage must comply with the currently adopted CBC/ASCE 7 provisions for
nonstructural components (Risk Category II, ASCE 7 Chapter 13). The design should confirm that anchor
spacing, embedment, and slab edge distances meet the required strength and deformation limits for the
cabinet’s mass and center of gravity. Wind load is not expected to govern since the cabinet is located within the
building equipment area.

Professional Opinion
Based on comparison with the previous approved analysis and the proposed 2,300-pound cabinet weight, it is
the professional opinion of M Squared Wireless that the modification will not increase structural demand and
will likely result in a net reduction in load effects on the existing structure. Provided that:
1. Anchorage is designed and installed per current code and manufacturer specifications.
2. The supporting slab or pad thickness, reinforcement, and concrete strength are verified to meet design
assumptions.
3. Seismic restraint and anti-overturn measures are provided in accordance with current code and
equipment manufacturer requirements.
Then the proposed battery cabinet replacement is expected to remain structurally adequate and code-
compliant.
