Type of construction: Per permit record, it’s type V-B construction – Please revise plan for type V-B construction. On the plan, please specify whether the building is equipped with fire-sprinkler or not. Please provide proof from OCFA. Sheet Index: MEP are under a separate review – Please note on plan “SEPARATE REVIEW” for MEP sheets. Sheet G02: On the plan, please clearly show an accessible route from the public sidewalk to the building primary accessible entrance. On the plan, please clearly show an accessible route from the accessible parking stall to the building primary accessible entrance. Accessible route shall be 5% maximum slope, in the direction of travel, and 2% maximum cross-slope. Cleary show curb and curb ramp with truncated dome. 2nd floor exit: The spiral stairway is not an exit access stairway – Please revise plan to show 2 exits are provided (not three). On the plan, please show an accessible route from the entrance to the second floor to the area of alteration. Existing elevator: please provide information for elevator to show conformance with Section 11B-407. Details shall be clearly cross-referenced onto the plan. Sheet A01: Restrooms: Please clearly reference to sheet A02 for the accessibility requirement. One-hour corridor within the tenant space: Please provide opening protection between the private check-out area and the corridor. Please provide opening protection between the file storage / reception and the private check-out area. Provide opening protection between sterilization / lab and the corridor.Provide opening protection between private office and corridor. Seating area: Please show wheelchair spaces (5% of the seats). Treatment rooms: Please show/provide minimum 36” clear along the treatment chair. Restroom door opening into the corridor – a 5-ft maneuver space is required on the pull side of door. Please provide wall construction details for the fire-rated wall assembly. Provide fire-rated assembly for corridor walls and ceiling. Details shall be clearly cross-referenced onto the plan. Detail 3/A02: Door schedule Fire-rated door – Please specify shelf-closing and latching. Please provide the following notes on plan: Egress doors/gates shall be readily openable from the egress ide without the use of a key or special knowledge or effort. (1010.1.9) Openable parts of such hardware shall be 34 inches minimum and 44 inches maximum above the finish floor or ground. The force for pushing or pulling open a door shall be 5-lb maximum. Operable parts shall be operable with one hand and shall not require tight grasping, pinching, or twisting of the wrist. Detail 2/A02: the accessible counter – Please revise accessible service counter to show 34” high maximum. Detail 1/A02: Single accommodated restroom shall be identified with a door symbol as follow: Sheet A03: Details provided on sheet A03 appeared inconsistence with the floor plan. Please revise details to show one-hour walls, corridor ceiling, etc.
