Exhibit A <br />SCOPE OF SERVICES <br />A. Provider shall conduct TKK Karate DO for children at various sites, ages 5 & up <br />B. Provider shall teach such or similar classes (1) at the times below at a facility to be <br />designated by the City or (2) on a schedule otherwise agreed upon by the parties for each <br />class session or term, including the location, specific days and hours when classes will be <br />held, and holidays to be observed, in accordance with City's needs. <br />TKK KARATE DO <br />Traditional Japanese Martial Arts program to help develop self-confidence and improve physical <br />conditioning while growing in the spirit of courtesy, effort, and discipline. Training techniques are designed <br />to make the body strong and hard. Quicken your hand, elbow and knee reflexes. The program is structured <br />so that students can train in the spirit of competition. <br />INSTRUCTOR: Santiago Bautista, AGES 5&Up <br />• TKK Karate DO will consist of 1'/2 hour, per day, 2 per week. <br />Location: El Salvador Center, 1825 W. Civic Center Dr, Santa Ana (714) 647-6558 <br />TKK Karate DO will consist of P/2 hour, per day, 1 per week. <br />Location: Memorial Center, 2102 S. Flower St., Santa Ana, (714) 571-4242 <br />TKK Karate DO will consist of 1 `/2 hour, per day, 2 per week. <br />Location: Salgado Center, 706 N. Newhope St., Santa Ana, (714) 571-4267 <br />C. Provider will provide and be responsible for equipment, records, and personnel and <br />cleanup of the facilities and materials necessary to ensure the safety and effectiveness of <br />said instruction. <br />CLASS SIZE <br />1) Each class must have a minimum of 1 paid student and no more than a maximum of 20/25. <br />2) No registration will be accepted after the second meeting of class. <br />3) In the event the minimum number of enrollees is not realized by the second meeting of the <br />class, the class shall be canceled. Provider will be under no obligation to provide services and the <br />City will have no obligations to pay Consultant compensation <br />CLASS FEES <br />1) Each participant shall pay class registration fee as established by City. <br />2) Provider may not waive class participation/registration fees. <br />3) Only registered participants may participate in class. <br />4) Any refunds to participants will be made in accordance with City policy. <br />5) Any material fee shall be established by mutual agreement of City and Provider and shall be <br />payable directly to Provider. <br />
