Exhibit A SCOPE OF SERVICES A. Provider shall conduct Judo/Jujitsu for Kids & Adults 6-70 years. B. Provider shall teach such or similar classes (1) at the times below at a facility to be designated by the City or (2) on a schedule otherwise agreed upon by the parties for each class session or term, including the location, specific days and hours when classes will be held, and holidays to be observed, in accordance with City's needs. JUDO JUJITSU JuDo Jujitsu class is a program which includes two workouts each week with exercising stretching, and learning self-defense. This program is designed for beginning to advance level students to get you into shape and introduce you to martial arts. INSTRUCTOR: Prof. Kevin Dalrymple, eighth degree black belt in Danzan Ryu jujitsu. LOCATION: Southwest Senior Center, 2201 W. McFadden Ave., Santa Ana, (714) 647-5306 • Judo/Jujitsu will consist of monthly session, held 2 days per week, 1 hour per day, ages 6-14 yrs. • Judo/Jujitsu will consist of monthly session, held 2 days per week, 1 hour per day, ages 14-70 yrs. C. Provider shall provide all materials, supplies, equipment, records and personnel. Providet shall be responsible for clean-up of the facilities and materials and shall ensure the safety and effectiveness of instruction. CLASS SIZE REGISTRATION A. All Judo/Jujitsu classes mentioned above must have a minimum of 2 paid students and no more than a maximum of 40 paid students. B. If the minimum registration has not been reached by the second class, the class may be cancelled by mutual agreement of Provider and City. Provider will be under no obligation to provide services for the cancelled classes, and the City will have no further obligations to pay Provider compensation for the remaining classes that were cancelled in that session. CLASS FEES A. Each participant shall pay class registration fees as established by City. B. Any materials fee shall be established by mutual agreement of City and Provider and shall be payable directly to Provider. C. Only registered participants may participate in class. D. Any refunds to participants will be made in accordance with City policy.
