Huizar <br />Re: Final Extension of Agreement for Provision of Water Ouality Testing <br />Services, Agreement No. A-2014-131 <br />Pursuant to Section 4 ("Term") of the above -referenced Agreement, entered into by Clinical <br />Laboratory of San Bernardino, Inc. and the City of Santa Ana, dated June 3, 2014, and as <br />previously extended and amended, the term of the Agreement is hereby extended for a final two <br />(2) year period, from June 3, 2018 through June 2, 2020, The insurance certificates are required <br />to be extended and/or renewed to cover this extension.
