Program goals are: <br />• Of those women who stay in the program for at least three months, 100%will obtain or maintain full time <br />employment; <br />• Of those same residents, 70% will moved into safe, affordable housing; <br />Of those women, 25%will move to appropriate second -stage housing, such as our Positive Step house; <br />Of the women who completed our required Financial Empowerment curriculum, 100%will demonstrate <br />increased understanding of their finances, including credit history and how to create a budget, and 65%will <br />actively save for their future. <br />'erformance Measures <br />WISEPIace collects and analyzes data on all of our residents and all programs: <br />A. We track and report the number of participants through county -wide Client Management Information System <br />(CMIS); <br />B. We track the number of women that obtain a job within 30 days of program entrance, through weekly client <br />notes and case management; <br />C. We track and report the number of residents who benefit from referrals for medical assistance, such as <br />psychological counseling, tuberculosis testing, HIV and AIDS training and counseling, vision and dental care, <br />mammograms, wellness health services, anger management, mental health screenings and referrals. <br />D. We track and report the number of residents who graduate from the program with a full-time job, savings <br />and a safe, affordable place to live, through CMIS data and exit interviews; <br />E. We measure long term success by surveying graduates 6 months and 12 months post-WISEPIace. <br />EXHIBIT A <br />
