Services Agreement <br />MW Prudential Provided by Prudential Retirement <br />Insurance and Annuity Company <br />Section 2. ELECTION OF ENHANCED SERVICES <br />The following optional Services have been elected by the Plan Sponsor. The Services will be <br />administered in accordance with the Administrative Procedures. Some Services are dependent upon <br />receipt of all required data in a manner acceptable to Prudential. <br />A. ADMINISTRATIVE & RECORDKEEPING SUPPORT <br />THE INDUSTRY TREND REPORT, which compares industry data, including statistics on various plan <br />features, services and Participant behaviors to your Plan. <br />AD-HOC REPORTING, to provide plan data and/or analysis through a specialized report, exhibit, or <br />data table that is not available through other plan administrative reports. <br />PARTICIPANT SUPPORT: <br />AGREED UPON NUMBER OF ADDITIONAL INSTRUCTOR -LED PARTICIPANT EDUCATION <br />WEBINARS. <br />CUSTOM COMMUNICATIONS which includes support by a communications strategist who will develop <br />and implement a Plan -specific communication strategy. Additional services may include onsite meetings, <br />custom websites or custom campaigns. Additional costs will apply to the actual services and tools <br />identified in the custom strategy. <br />PLAN DOCUMENT SERVICES: <br />DISCRETIONARY PLAN AMENDMENTS AND RESTATEMENTS, for Plan Sponsor- <br />initiated/discretionary changes using Prudential's specimen document. <br />CONSULTING SERVICES, Plan -specific guidance and consultation on a series of complex topics, <br />including but not limited to merger & acquisition, Plan design & analysis, DOL/IRS correction programs, <br />fiduciary responsibilities and 404(c) compliance. Projects requiring a formal proposal will include a <br />statement of work and estimate of the charges, delivered prior to the commencement of the work. <br />B. INVESTMENT ADMINISTRATION <br />INVESTMENT COMMITTEE MEETING SUPPORT delivered via phone. <br />W <br />25D-21 <br />
