24 of 45 <br />Additionally, your complete application must include the following narratives and non-form <br />attachments. <br />a. MOU between the PHA, PCWA and CoC. Requirements of the MOU are explained in Section <br />III.C.8. of this NOFA. <br />b. Statement of Need. The Statement of Need must be signed by the Executive Director, Chief <br />Executive Officer, or individual of equivalent position, of the PCWA, even if the PCWA has <br />authority under applicable State law to contract out its functions to another organization. The <br />Statement of Need must also be signed by the CoC Board Chair and CoC Collaborative Applicant <br />Authorized Representative. <br />The Statement of Need must adequately demonstrate the need for FUP vouchers in the PHA’s <br />jurisdiction, which is not being met through other existing programs. This is done by indicating <br />the number of FUP-eligible families who currently have children in danger of being placed in out- <br />of-home care or who cannot be returned from out-of-home care due to inadequate housing. <br />Additionally, the Statement must include the number of youths who have attained 18 years and <br />not more than 24 years of age and who have left foster care, or will leave foster care within 90 <br />days, in accordance with a transition plan described in section 475(5)(H) of the Social Security <br />Act, and are homeless or at risk of becoming homeless at age 16 or older. The Statement must <br />identify separately the number of FUP-eligible youth in the community who are no longer part of <br />the PCWA's active caseload (e.g., youth that left foster care who are now homeless or at-risk of <br />homelessness). The CoC must contribute to the identification of FUP-eligible youth that are <br />experiencing homelessness in the community and who are no longer part of the PCWA's active <br />caseload through integration into the local CoC(s)' coordinated entry process. <br />The Statement of Need must also specify the number of vouchers projected to be needed to assist <br />FUP-eligible families and FUP-eligible youths over a 12-month period and describe how this <br />estimate was calculated. The Statement of Need shall be a maximum of five pages long. <br />c. Required Documentation for Rating Criteria. Those applicants who will apply for rating criteria <br />points must carefully review the documentation requirements of each rating criteria (see Section <br />V.A.1. of this NOFA) and submit the required documentation with their application materials for <br />each rating criteria they are applying for. <br />d. PCWA Contractor Documentation. If the PCWA has authority under State law to contract out <br />its function to another organization and the PCWA has exercised this authority, the application <br />under this NOFA must contain a one page letter signed by the Executive Director, Chief <br />Executive Officer, or individual of equivalent position of both the PCWA and the contractor <br />organization which indicates that this agreement has been executed, including the name of the <br />contractor organization and the applicable State law authorizing such an agreement. <br />2. Format and Form. <br />Narratives and other attachments to your application must follow the following format guidelines. <br /> <br /> <br />C.
