Description of Work - In the space below, describe the program to be funded during the 12 -month contract <br />period. What specific activities will be undertaken during the contract period. Please be concise in your response. <br />Only the viewable space will print. <br />"Illumination Foundation (IF) will use ESG funds to provide homeless prevention assistance to families <br />residing in Santa Ana who are in jeopardy of losing housing. The funds will be used to provide families <br />with rent assistance in order for families to maintain their current housing without the risk of being <br />homeless. Through referrals and outreach activities, IF staff identify homeless persons in need of <br />housing services and connect them to our rapid re -housing and homeless prevention services. <br />Funding allocations will include: Security Deposit Assistance: Provide a one-time deposit assistance <br />payment and Short <br />Term Rental Assistance: Short-term rental assistance (1-3 months). <br />In addition, families will receive assistance with application fees, security deposit assistance if <br />necessary, first and last month's rent, and move in costs for clients in the program, if they need to be <br />relocated. <br />Other essential services that will be provided along with rental assistance and/or affordable housing <br />placement include life skills classes, workforce development and job readiness training classes, financial <br />literacy classes, medical services, behavioral health assessments, childhood enrichment programs, and <br />connections to social services and other community resources. <br />The Illumination Foundation Homeless Prevention Program will help the Santa Ana community reduce <br />the size of the <br />homeless population by providing assistance and case management services that can aid families in <br />preserving their independence. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12 -month <br />contract period per quarter.
