<br />The plan should begin being addressed through the provisions of available in -
<br />custody programming and should continue when offenders transition from jail
<br />incarceration. Part of the reentry services program work would involve the
<br />development of a discharge plan and process to aid the offenders transition from jail
<br />to the community and to sustain gains made through pre-release interventions. The
<br />discharge process should begin at least 30 — 45 days prior to release and be created
<br />utilizing motivational interviewing techniques allowing for the offender to be the key
<br />person in the development of their own discharge plan. Examples of discharge
<br />interventions include: employment class, scheduled appointments in the community,
<br />a temporary supply of medication (as needed), identification documents,
<br />transportation to a service provider, home or parole office, resource packets, and
<br />contact information for key individuals who will facilitate the individual service plan in
<br />the community.
<br />Work done while in jail, including treatment, relationships with service providers, and
<br />connecting individuals to service appointments in the community should continue
<br />upon release from jail through the assistance of the Jail reentry staff assigned to the
<br />Program. It is important in developing the reentry program that staff devote time and
<br />work to organizing selected community-based organizations and support networks to
<br />provide a continuity of care process — or in many cases, initiate care — through
<br />services, training, treatment, and case management when an individual is released.
<br />Examples of community-based interventions include services in areas such as job
<br />readiness training, substance abuse treatment, mental health counseling, post -
<br />release case management, access to reentry information through outreach that
