The materials that comprise the facilities. <br />E. A description of any ground disturbance necessary to <br />complete the proposed project. <br />2. Detailed plans and design drawings of the proposed installation, <br />including a detailed description of the existing wireless facility and <br />of the modifications that are proposed to be made to the existing <br />wireless facility; the exact location of the proposed facility; the <br />dimensions (including the volume) and specific location of the <br />wireless facility and its elements, including but not limited to any <br />accessory equipment that will be installed. The description must <br />Include detail on any excavations and deployment outside the <br />current site, including any physical, wireline, interconnections to <br />other locations, noting existing wireline connections and new <br />ones. <br />A copy of all approvals and/or permits for the tower or base <br />station that is to be modified, and any subsequent modification <br />applications and permits, and of any required conditions (imposed <br />by the City and/or third party) placed on the initial or subsequent <br />permits. <br />4. A report signed by a California licensed professional engineer <br />specializing in structural engineering, containing the following: <br />A. In the case of an attached wireless facility, documentation <br />of the ability of the structure to accept the antennas, the <br />proposed method of affixing the antennas, and the precise <br />point at which the antennas shall be mounted. <br />B. In the case of a wireless facility with a support structure <br />(e.g. monopole), documentation that the structure is <br />capable of supporting the antennas and complies with <br />applicable laws and codes, as well as the structure's <br />capacity for additional collocated antennas, and the precise <br />point at which the antennas shall be mounted. <br />Sec. 33-237. - Distributed Antenna System (DAS). <br />Applications for wireless facilities comprising a DAS shall be submitted <br />simultaneously and each individual location within the system shall be processed and <br />considered for approval separately.
