ji <br />KMEA_ <br />• A written statement of that indicates the person who conducted the survey is either a State Certified Asbestos Consultant <br />(CAC) and/or a State Certified Site Surveillance Technician (SST) working under the direction of a CAC. <br />• The dates the survey was conducted. <br />• A listing of all suspected materials containing any asbestos, a listing of all samples collected, and a sketch of where the <br />samples were taken. <br />• The name, address, and telephone number of any laboratory used to conduct analyses of materials for asbestos content. <br />• A statement of the Laboratory qualifications that will indicate that they used the analytical Method specified in Appendix A, <br />Subpart F, 40 CFR Part 763, Section 1, Polarized Light Microscopy and that such laboratory is accredited by the National <br />Voluntary Laboratory Accreditation Program (NVLAP). <br />• In addition to describing the laboratory analytical method(s) used, sampling protocols, test data, and any other information <br />used to identify or quantify any materials containing asbestos will be provided. <br />• A general description of the condition of the facility, including but not limited to a description of any obvious fire or structural <br />damage. <br />• A summary of the survey findings and related regulatory requirements. <br />• Conclusions and recommendations based on the report findings. <br />Lead -Based Paint Surveys <br />KMEA's lead survey will be conducted in accordance with the California Code of Regulations, Title 17, Section 35001 et seq and 8 <br />CCR 15321 and will focus on identifying lead containing coatings, paints and ceramic tiles that would require removal prior to <br />demolition to comply with Cal -EPA Department of Toxic Substances Control (DTSC) hazardous waste disposal regulations. Survey <br />will also address Cal/OSHA requirements involving disturbance and handling of identified lead containing materials. Survey methods <br />will include collecting paint chip samples and submitting such samples for laboratory analysis. <br />The survey and report will include the following information: <br />• Executive summary. <br />• The name, address, and telephone number of the person who conducted the survey. <br />• A written statement of that indicates the person who conducted the survey is a California Lead -Related Construction <br />Certified Inspector/Risk Assessor. <br />• The dates the survey was conducted. <br />• A listing of all suspected materials containing any lead, a listing of all samples collected, and a sketch of where the samples <br />were taken. <br />• The name, address, and telephone number of any laboratory used to conduct analyses of materials for lead content. <br />• A statement of the laboratory qualifications that will indicate that they used Flame Atomic Absorption (FLAA) or Total <br />Threshold Limit Concentration (TTLC) in accordance with EPA method SW 846. Laboratory will successfully participate in <br />the AIHA ELLAP Industrial Hygiene Laboratory Program. <br />• In addition to describing the laboratory analytical method(s) used, sampling protocols, test data, and any other information <br />used to identify or quantify any materials containing lead will be provided. <br />• A summary of the survey findings and related regulatory requirements. <br />• Conclusions and recommendations based on the report findings. <br />General Assumptions <br />KMEA makes the following general assumptions associated with preparing the Phase I ESAs, Phase II ESAs and conducting <br />Hazardous Materials Surveys. These assumptions apply to the scope of services and schedule outlined in Section 2 of our proposal <br />in response to the sample site locations identified in the City RFP No. 18-043. Additional project- and scope -specific assumptions <br />are also presented in Section 2. <br />• KMEA will have unencumbered access to the site. <br />• To the extent feasible, the City will collaborate with KMEA in identifying knowledgeable site contact(s) to be interviewed <br />and to schedule and coordinate the Phase I ESA site visit and provide access to survey areas and sampling locations during <br />Phase II ESAs and Hazardous Materials Surveys. <br />City of Santa Ana Page 10 June 5, 2018 <br />Various Environmental Services <br />RFP NO.: 18-043 <br />
