Exhibit A SCOPE OF SERVICES A. Provider shall conduct ESL classes for Adults. B. Provider shall teach such or similar activities at the times provided below at facilities to be designated by the City, Scheduling is provided below for the listed activities session or term, including location, specific days and hours when activities will be held, and holidays to be observed, in accordance with City's needs. INSTRUCTOR: Leif Larsen LOCATION: El Salvador Center, 1825 W Civic Center St., Santa Ana, (714) 647-6558 DAY TIME. COST Thurs 7:00 P.M - 8:00 P.M $10 per week C. Provider shall provide all materials, supplies, equipment, records and personnel. Provider shall be responsible for clean-up of the facilities and materials and shall ensure the safety and effectiveness of instruction. D. Provider will allow one week in April, August and December for student grading instead of class time. CLASS SIZE A. Each class must have a minimum of 3 paid students and no more than 18 students. B. No registration will be accepted after the second meeting of classes. C. If the minimum registration has not been reached by the second class, the class shall be canceled. Provider will be under no obligation to provide services for the cancelled classes, and the City will have no further obligations to pay Provider compensation for the remaining classes that were cancelled in that session. CLASS FEES A. Each participant shall pay class registration fees as established by City. B. Provider may not waive class participation/registration fees. C. Only registered participants may participate in class. D. Any refunds to participants will be made in accordance with City policy. E. Any materials fee shall be established by mutual agreement of City and Provider and shall be payable directly to Provider.
