ARTICLE XI <br />11.0 EMPLOXEE INSURANCE <br />11.1 Health Insurance. Part Time Civil Service employees shall be eligible to participate, <br />in the City's insurance programs as outlined below. <br />A. The City will contribute 75% or depending on the individual employees' <br />selection of CaIPERS health insurance the entire premium, whichever is less, of <br />the City contribution allotted to full time SEIU members for health insurance <br />premium costs for the employee and any dependents. <br />B. The City will contribute 75% or depending on the individual employees' <br />selection of dental insurance, the entire premium, whichever is less, of the City <br />contribution allotted to full time SEIU members for dental insurance premium <br />costs for the employee and any dependents. <br />C. An employee who is covered under a spouse's non -city sponsored health plan <br />and voluntarily waives, in writing, their City paid health insurance coverage <br />will receive a cash payment each month in an amount equal to 75% of the full <br />time adjusted monthly premium reimbursement for the City's lowest <br />"employee only" coverage. Employees must show proof of qualified group <br />health insurance coverage. <br />11.2 Long -Term •Disability_(L T.D.) Insurance. The City shall allow employees, for the <br />term of this Agreement, to participate in any aspect of the Plan offered to"the City <br />at the City rate. The cost for this benefit shall be borne by the employee. <br />113 Life Insurance. The City shall allow employees, for the term of this Agreement, to <br />participate in any aspect of the plan.offered to the City at the City rate. The cost <br />for this benefit shall be borne by the employee. <br />11,4 Vision Plan. The City shall offer a non-contributory vision plan through payroll <br />deduction for employees wishing to participate in such a plan. <br />11.5 Insurance Committed. When there is a need to discuss matters relating to <br />employee insurance and the City believes it would be beneficial to involve an <br />Insurance Committee, the Union shall have an equal number of representatives as <br />the City on such a committee to meet as necessary. <br />31 <br />
