Complete Answers & Responses <br />All bid proposals must provide answers to all questions and provide complete <br />responses to each and every requested item and category as specified in <br />Section 7. Specific submission data have been provided for your convenience for <br />each submission item. Proposers are required to supply, at a minimum, the items <br />listed for specific submission. Proposers are permitted, within the prescribed <br />page limits, to submit additional information that they deem helpful in the <br />evaluation and grading process. Failure to provide answers to all questions and <br />complete responses to all requested item categories may result in <br />disqualification, at the City's discretion; <br />G. Table of Contents <br />Bid proposals must include a table of contents listing the individual information as <br />requested by section and set forth in Section 7, Bid Proposal Submission Forms, <br />and their corresponding page numbers. Tabs or dividers must be used to clearly <br />separate each of the individual sections; <br />H. Cover Letter <br />Bid proposals must include a cover letter on official letterhead describing the <br />Proposer, and must include the following information: <br />i. The official name of the Proposer, and stating the Proposer's interest in <br />participating in the RFP; <br />ii.
