Complete the 911/Fire/EMS Contracts chart found in the following page and include <br />the following information: <br />• List the city(ies) or Fire Department(s) Agencies for which provider has provided <br />emergency ambulance transportation services during the past ten (10) years that <br />can be used to meet one of the "experience" requirement in the RFP.
