ENDORSEMENT #010 <br />This endorsement, effective 12:01 a.m., October 1, 202forms a part of <br />Policy No. RAD5000476 issued to Falck USA, Inc. <br />by Greenwich Insurance Company. <br />THISENDORSEMENTCHANGESTHEPOLICY.PLEASEREADITCAREFULLY <br />ADDITIONALINSURED–PRIMARYANDNONCONTRIBUTORY–DESIGNATEDCONTRACT <br />This endorsement modifies insurance provided under the following: <br />BUSINESS AUTO COVERAGE FORM <br />MOTOR CARRIER COVERAGE FORM <br />AUTO DEALERS COVERAGE FORM <br />Withrespecttocoverageprovidedbythisendorsement,theprovisionsoftheCoverageFormapply <br />unless modified by the endorsement. <br />Schedule <br />Work <br />Additional Insured(s) <br />The City of Santa Ana (“City”) and Orange County9-1-1 Emergency Transportation Services as <br />Fire Authority (“OFCA”), and their respective specifiedunder the Agreement between the <br />officers, officials, employees, representatives, and Insured and Additional Insured(s), including both <br />the City or OFCA designated volunteers.the First Tier Service and the Mutual Aid Service. <br />COVEREDAUTOSLIABILITYCOVERAGE,WhoIsAnInsured,isamendedtoincludeasan“insured” <br />thepersonororganizationlistedintheScheduleabove,butonlywithrespecttoliabilityfor“bodilyinjury” <br />or"propertydamage"otherwisecoveredunderthispolicycaused,inwholeorinpart,bythenegligent <br />acts or omissions of: <br />1.You, while using a covered “auto”; or <br />2.Anyotherperson,excepttheadditionalinsuredoranyemployeeoragentoftheadditional <br />insured, operating a covered “auto” with your permission; <br />in the performance of your work as described in the Schedule above. <br />SectionIV–BusinessAutoConditions,BGeneralConditions,5.OtherInsuranceisamendedtoaddthe <br />followingfortheAdditionalInsurednamedinthisendorsementonly.Thisinsuranceisprimary <br />andwillnotseekcontributionfromanyotherinsuranceavailabletotheadditionalinsured.Thisis <br />subjecttotheadditionalinsuredbeinganamedinsuredundersuchotherinsuranceandyou <br />haveagreedinwritinginacontractoragreementthatthisinsurancewouldbeprimaryandwould <br />not seek contribution from any other insurance available to the additional insured. <br />InnoeventshallanypersonororganizationlistedintheSchedulebecomean“insured”pursuanttothis <br />MANUS Page 1 of 2 <br />© 202 X.L.
