City of Santa Ana <br />Documents (e.g., Quarterly Response Time Reports, CQI, etc.) and <br />that are not submitted on time may result in the imposition of a $500 <br />penalty per occurrence. <br />C. Corrective Action Plan <br />Situations which come to the attention of, or are reported to, the OCFA <br />EMS Section Battalion Chief and which appear to constitute a Contractor <br />service or performance deficiency or substantial inadequacy, as <br />determined by the OCFA, shall be immediately investigated by the OCFA. <br />An example of such situation might be the Contractor's failure to achieve <br />at least a 90% response time performance in any single Code category for <br />a quarterly reporting period. At the discretion of the OCFA, a Corrective <br />Action Plan may be imposed on the Contractor to correct identified and <br />verified performance deficiencies and inadequacies. The OCFA <br />authorized representative shall meet to develop a written Corrective Action <br />Plan (CAP) within fifteen (15) working days of the identification and <br />verification of the service or performance deficiency, or substantial <br />inadequacy, in accordance with the following CAP requirements: <br />i. CAP Format <br />The CAP shall describe the following: <br />(1) The service or performance deficiency, or substantial inadequacy <br />shall be identified; and <br />(2) The method by which Contractor is to correct the service or <br />performance deficiency, or substantial inadequacy, shall be <br />outlined. Contractor shall sign the CAP, thereby agreeing to the <br />corrective action set forth in the CAP, with any areas of <br />disagreement noted in writing. A copy of the signed CAP shall be <br />furnished to Contractor at the conclusion of the CAP meeting. <br />ii.
