E. EXHIBIT F — CERTIFICATION OF NONDISCRIMINATION BY PROPOSER <br />The Proposer shall submit a statement that it does not discriminate in its employment actions <br />or services rendered, as per California and Federal law. <br />F. EXHIBIT G — RESPONSIBLE PROPOSER — SUPPLtMtN IAL Wit, <br />The Proposer'shall submit a detailed SAF with all questions answered. . <br />G. EXHIBIT H — NONCOLLUSION AFFIDAVIT i <br />The Proposer shall submit a statement whereby Proposer declares .that the proposal is not <br />made in the interest of, or on behalf of, any undisclosed person,j partnership, company, <br />association, organization, or corporation; and that the proposal is genuine and not collusive or <br />sham. <br />H. EXHIBIT I — <br />Proposer shall submit an additional insured endorsement as per Exhibit I. <br />i <br />The proposal must be completely responsive to the RFP. Any exceptions to the <br />requirements of this RFP must be included in the proposal. Such exceptions must be included <br />as a separate element of the proposal entitled "Exceptions and Deviations." The Executive <br />Director of Human Resources, in his sole and absolute discretion, may authorize or deny any <br />exceptions. <br />J. The City reserves the right to reject any or all proposals submitted and no representation is <br />made hereby that any contract will be awarded pursuant to this RFP or otherwise. The City <br />reserves the right to waive minor inconsistencies in submitted Proposals. <br />XV. CONSULTANT SELECTION — PROPOSAL AND EVALUATION <br />Proposal should be straightforward, concise and provide "layman" explanations of technical terms <br />that are used. Emphasis should be concentrated on conforming to the RFP instructions, responding <br />to the RFP requirements, and on providing a complete and clear description of the offer. Proposals <br />which appear unrealistic in terms of technical commitments, lack of technical competence or are <br />indicative of failure to comprehend the complexity and risk of this agreement may be rejected. <br />The response to this RFP for Medical Services Review program should contain documentation of <br />consultants and/or consultant's Department's credentials and expertise. Substantial consideration will <br />be given to Consultants with demonstrable and documented experience in similar work. Responses <br />will be evaluated on the following basis: <br />25% Qualifications and Experience <br />25% Understanding of Scope of Work <br />25% Demonstrated Knowledge of Medical Services Review program management <br />10% Cost competitiveness <br />10% Staff Availability <br />5% Responsiveness of the Proposing Firms and Fee Schedules <br />The responses received will be reviewed by an evaluation committee appointed by the Executive <br />f of Santa Ana Human Resources Department <br />for Medical Services Review — August 27, 2018 <br />26E=l 3 <br />
