ARTICLE XI <br />I L0 EMPLOYEE INSURANCE <br />11.1 Health Insurance. The City shall contribute toward the payment of premiums for affected <br />employees and their dependents tinder the existing health insurance programs or new <br />program or programs providing substantially similar or improved coverage and benefits <br />selected in accordance with procedures in effect on the effective date of this MOU: <br />A. Effective January 1, 2018, the City's monthly contributions for medical insurance <br />shall up to be as follows: <br />EMPLOYEE <br />up to $654.00 <br />EMPLOYEE+1 <br />up to $1,307,00 <br />FAMILY <br />up to $1,700.00 <br />B. Any contribution necessary to maintain benefits under said medical plans in excess <br />of the amount set forth above shall be borne entirely by the employee, <br />C. An employee who is covered tuider a spouse's non -City sponsored health plan and <br />voluntarily waives, in writing, their City paid health insurance coverage will receive <br />a cash payment each month in an amount equal to 50% of the total monthly <br />premium amount for the City's lowest "employee -only" coverage. Employees must <br />show proof of qualified group health insurance coverage. <br />11.2 Dental Insurance. The City shall contribute a maximum amount of tip to ninety ($90) <br />dollars per month per employee toward the payment of premiums for denim insurance plans <br />provided by the City for employees covered by this MOU and their eligible <br />dependents. Any contribution necessary to maintain benefits under said dental plans in <br />excess of the amount set forth above shall be borne entirely by the employee. <br />I L3 Lone -Term Disability (LTD) Insurance. The City shall contribute up to ten dollars and <br />thirty-nine cents ($10,39) per month to employees enrolled in the 130 day elimination <br />period LTD plan. For employees who elect to upgrade their LTD benefit coverage to a 60 <br />day elimination period plan, the City will contribute up to eleven dollars ($11.00) per <br />month. Any amount necessary to maintain benefits under the long term disability <br />insurance plans provided by the City in excess of the amounts set forth above shall be borne <br />by the employee. <br />11.4 Life Insurance. The City shall provide its employees with a thirty thousand dollar <br />($30,000) term life insurance policy and a thirty thousand dollar ($30,000) Accidental <br />Death and Dismemberment (AD&D) coverage at no cost to the employee. <br />46 <br />
