ARTICLE XII <br />12.1 During the term of this MOU, the City will contribute toward the payment of premiums <br />for health, dental and long-term disability insurance plans administered by the <br />Association for the benefit of the employees represented by the Association as follows: <br />A. Medical Insurance. The City will contribute $1,510.00 per month ($1,575.00 <br />effective July 1, 2019) to the Santa Ana Police Officers Association Medical <br />Insurance Trust Fund for each affected employee enrolled in medical insurance <br />plans provided by the Association for its bargaining unit members and their <br />eligible dependents. Any contributions necessary to maintain benefits under the <br />plans in excess of the aforementioned amount per month shall be home entirely <br />by the Association and/or the enrollee. Members of the bargaining unit who are <br />not Association members and therefore not eligible to purchase insurance through <br />the Trust will receive from the City up to $1,575.00 per month to be used only for <br />the purchase of medical insurance for themselves and eligible dependents. <br />The Association will provide the City with proof that each of the employees in the <br />Association who have enrolled in insurance are receiving insurance. The <br />Association will also provide the City (once each quarter) with a list of the group <br />insurance plan and rates for medical, dental and Long Term Disability. <br />B. Life Insurance. The City shall maintain in effect for the term of this MOU its <br />existing life insurance plan for employees covered by this MOU on the same basis <br />as said plan was.offered to employees as of December 31, 1988 except the term <br />life insurance coverage for each affected employee will be in the amount of thirty <br />thousand dollars ($30,000), plus twenty thousand dollars ($20,000) accidental <br />death and dismemberment (AD + D) coverage, at no cost to the employee. <br />12.2 Retiree Health Contribution. <br />All employees covered by this MOU will contribute two percent (2%) of their regular <br />rate of pay through payroll deduction to a fund maintained by the Santa Ana Police <br />Officers Association for the purpose of providing retiree health insurance premium <br />reduction assistance. <br />Effective October 1, 2018, and on October 1 of each year the City shall contribute an <br />amount equal to two percent (2%) of each employee's regular rate of pay (base pay plus <br />additional forms of pay paid each pay period) of each active employee who is covered by <br />this MOU as of October 1 of each year to a fund maintained by the Santa Ana Police <br />Officers Association for the purpose of providing retiree health insurance premium <br />reduction assistance. <br />12.3 Employees on unpaid leave of more than fifteen (15) calendar days shall not receive the <br />City's contribution toward any insurance coverage.
