Exhibit A <br />SCOPE OF SERVICES— Traditional Japanese Martial Arts/Santiago Bautista <br />A. Provider shall conduct Traditional Japanese Martial Arts for children at various sites, age 7 & up <br />B. Provider shall teach such or similar classes (1) at the times below at a facility to be designated by <br />the City or (2) on a schedule otherwise agreed upon by the parties for each class session or term, <br />including the location, specific days and hours when classes will be held, and holidays to be <br />observed, in accordance with City's needs. <br />• For Each Class Session -Traditional Japanese Martial Arts will consist of I hour, per day, per <br />week, ages 7 & up. (EI Salvador Ctr) <br />• For Each Class Session -Traditional Japanese Martial Arts will consist of 1 hour, per day, per <br />week, ages 7 & up. (Memorial Center) <br />• For Each Class Session -Traditional Japanese Martial Arts will consist of 1 hour, per day, per <br />week, ages 7 & up. (Salgado Ctr) <br />C. Provider will provide and be responsible for equipment, records, and personnel and cleanup of the <br />facilities and materials necessary to ensure the safety and effectiveness of said instrvction. <br />D. If Provider allows others to teach his/her class, those teachers must be over 21, have obtained and <br />maintain an instructor rating, and be covered by Providers' insurance. Provider shall provide City <br />with documentation to verify instructor and insurance requirements. Black -belt rated students <br />may warm up beginning students, however, Provider must be present at all times. <br />CLASS SIZE <br />1) Each class must have a minimum of 5 paid students and no more than a maximum of 25. <br />2) No registration will be accepted after the second meeting of class. <br />3) In the event the minimum number of enrollees is not realized by the second meeting of the class, the <br />class shall be canceled. Consultant will be trader no obligation to provide services and the City will have <br />no obligations to pay Consultant compensation <br />CLASS f +ES <br />A. Each participant shall pay class registration fees as established by City. <br />B. Provider may not waive class participation/registration fees. <br />C. Only registered participants may participate in class. <br />E. Any refunds to participants will be made in accordance with City policy. <br />F. Any materials fee shall be established by mutual agreement of City and Provider and shall be <br />payable directly to Provider. <br />7 <br />
