Exhibit A SCOPE OF SERVICES A. Provider shall conduct TKK Karate Do for children at various sites, ages 5 & up B. Provider shall teach such or similar classes (1) at the times below at facilities to be designated by the City or (2) on a schedule agreed upon by the parties for each class session or term, including the location, specific days and hours when classes will be held, and holidays to be observed, in accordance with C,ity's needs. TKK KARATE DO Traditional Japanese Martial Arts program to help develop self-confidence and improve physical conditioning while growing in the spirit of courtesy, effort, and discipline. Training techniques are designed to make the body strong and hard. Quicken your hand, elbow and knee reflexes. The program is structured so that students can train in the spirit of competition. INSTRUCTOR: Santiago Bautista, AGES 5 & Up • TKK Karate DO will consist of 1 i/z hour, per day, 2 per week. Location: El Salvador Center, 1825 W. Civic Center Dr., Santa Ana (714) 647- 6558 TKK Karate DO will consist of 1 %= hour, per day, 1 per week. Location: Memorial Center, 2t02 S. Flower St., Santa Ana, (7t4) 571-4242 TKK Karate DO will consist of I Y hour, per day, 2 per week. Location: Salgado Center, 706 N. Newhope St., Santa Ana, (714) 571-4267 C. Provider will provide and be responsible for equipment, records and personnel. Provider shall be responsible for clean-up of the facilities and materials and shall ensure the safety and effectiveness of instruction. CLASS SIZE A. Each class must have a minimum of 1 paid student and no more than 25 students. B. No registration will be accepted after the second meeting of classes. C. If the minimum registration has not been reached by the second class, the class shall be canceled. Provider will be under no obligation to provide services for the cancelled classes, and the City will have no further obligations to pay Provider compensation for the remaining classes that were cancelled in that session. CLASS FEES A. Each participant shall pay class registration fees as established by City. B. Provider may not waive class participation/registration fees. C. Only registered participants may participate in class. D. Any refunds to participants will be made in accordance with City policy. E. Any materials fee shall be established by mutual agreement of City and Provider and shall be payable directly to Provider.
