Exhibit A <br />SCOPE OF SERVICES <br />A. Provider shall conduct TKK Karate Do for children at various sites, ages 5 & up <br />B. Provider shall teach such or similar classes (1) at the times below at facilities to be <br />designated by the City or (2) on a schedule agreed upon by the parties for each class <br />session or term, including the location, specific days and hours when classes will be held, <br />and holidays to be observed, in accordance with C,ity's needs. <br />TKK KARATE DO <br />Traditional Japanese Martial Arts program to help develop self-confidence and improve <br />physical conditioning while growing in the spirit of courtesy, effort, and discipline. <br />Training techniques are designed to make the body strong and hard. Quicken your hand, <br />elbow and knee reflexes. The program is structured so that students can train in the spirit <br />of competition. <br />INSTRUCTOR: Santiago Bautista, AGES 5 & Up <br />• TKK Karate DO will consist of 1 i/z hour, per day, 2 per week. <br />Location: El Salvador Center, 1825 W. Civic Center Dr., Santa Ana (714) 647- <br />6558 <br />TKK Karate DO will consist of 1 %= hour, per day, 1 per week. <br />Location: Memorial Center, 2t02 S. Flower St., Santa Ana, (7t4) 571-4242 <br />TKK Karate DO will consist of I Y hour, per day, 2 per week. <br />Location: Salgado Center, 706 N. Newhope St., Santa Ana, (714) 571-4267 <br />C. Provider will provide and be responsible for equipment, records and personnel. Provider <br />shall be responsible for clean-up of the facilities and materials and shall ensure the safety and <br />effectiveness of instruction. <br />CLASS SIZE <br />A. Each class must have a minimum of 1 paid student and no more than 25 students. <br />B. No registration will be accepted after the second meeting of classes. <br />C. If the minimum registration has not been reached by the second class, the class shall be <br />canceled. Provider will be under no obligation to provide services for the cancelled classes, <br />and the City will have no further obligations to pay Provider compensation for the <br />remaining classes that were cancelled in that session. <br />CLASS FEES <br />A. Each participant shall pay class registration fees as established by City. <br />B. Provider may not waive class participation/registration fees. <br />C. Only registered participants may participate in class. <br />D. Any refunds to participants will be made in accordance with City policy. <br />E. Any materials fee shall be established by mutual agreement of City and Provider and shall <br />be payable directly to Provider. <br />
