EXHIBIT 2 <br />Applicants signing the SF424 cover page either through electronic submission or in paper copy <br />submission (for those granted a waiver) affirm that the certifications and assurances associated <br />with the application are material representations of the facts upon which HUD will rely when <br />making an award to the applicant. If it is later determined that the signatory to the application <br />submission knowingly made a false certification or assurance or did not have the authority to <br />make a legally binding commitment for the applicant, the applicant may be subject to criminal <br />prosecution, and HUD may terminate the award to the applicant organization or pursue other <br />available remedies. Each applicant is responsible for including the correct certifications and <br />assurances with its application submission, including those applicable to all applicants, those <br />applicable only to federally -recognized Indian tribes, and those applicable to applicants other <br />than federally -recognized Indian tribes. All program -specific certifications and assurances are <br />included in the program Instructions Download on Grants.gov. <br />3. Lead Based Paint References <br />When providing housing assistance funding for purchase, lease, support services, operation, or <br />work that may disturb painted surfaces, of pre -1978 housing, you must comply with the lead- <br />based paint evaluation and hazard reduction requirements of HUD's lead-based paint rules <br />(Lead Disclosure; and Lead Safe Housing (24 CFR part 35)), and EPA's lead-based paint rules <br />(e.g., Repair, Renovation and Painting; Pre -Renovation Education; and Lead Training and <br />Certification (40 CFR part 745)). <br />V. Application Review Information. <br />A. Review Criteria. <br />1. Rating Factors. <br />In addition to meeting all of the threshold requirements of the NOFA, applicants can receive up <br />to 100 points by addressing the rating criteria specified in this section. <br />Applicants may also qualify for two additional bonus points if they have been approved for <br />participation in the FUP and Family Self -Sufficiency Demonstration (FUP FSS Demonstration). <br />Applicants qualifying for the additional two bonus points can receive up to 102 total points. <br />The rating criteria and bonus points that applicants receive will be used to rank the applicants <br />for the purposes of funding. (See Section V.B. of this NOFA for additional information on the <br />review and selection process.) <br />The minimum score for an application to be considered for funding is 61 points. However, <br />should HUD determine that no eligible applicants have scored at least 61 points, HUD may <br />award funds to an applicant scoring below 61 points. <br />PCWA's commitment to FUP-eligible youth Maximum Points: 15 <br />Scoring. Assign 15 points if the PCWA will commit staff resources to identify and refer, on at <br />least a quarterly basis, FUP-eligible youth in the community who are no longer part of its active <br />caseload (e.g., youth that left foster care who are now homeless or at -risk of homelessness). <br />30 of 45 <br />3-42 <br />
