The program follows the best -practice OERU model we have helped establish: <br />Outreach: Certified Enrollment Counselor staff (CEC) offer culturally and linguistically appropriate <br />information in hard -to -reach, low-income, and minority communities through partner sites, events, material <br />distribution, and a toll -free phone number. <br />Enrollment: CEC Staff meet in person with individuals and families to gather eligibility <br />information for 18 different health care and social services programs and submit households' <br />applications through the One-e-App or CaIHEERS, if qualified. Programs include Medi-Cal, Covered <br />California, Kaiser Child Health Plan, CalFresh, and CalWORKs, among others. <br />Retention and Utilization: Through our "Care Connections" case management, Health Access staff help <br />enrolled clients use and maintain benefits and other services. Educating residents on the importance of <br />preventive care, we assist with verifying eligibility, selecting a primary care provider, making appointments, <br />and completing annual renewals. <br />We also conduct "Know Your Benefits" workshops, which equip attendees with the knowledge and <br />confidence to navigate the health system. Topics include the importance of a primary care provider, how to <br />find a trusted doctor, how to prepare for a visit, and how to ensure continued coverage year to year —critical <br />issues for participants who may have secured health coverage for the first time. <br />Client services under CHIOC's Community Health Access Program are currently available Monday <br />through Saturday. In addition to regular office hours, we engage in 10-15 outreach presentations and events <br />each month; these are generally held on evenings and weekends. With current CDBG funding CHIOC added <br />enrollment availability on Saturdays from 10:00am to 2:00pm. <br />Services are typically provided in a one-on-one setting. However, CHIOC's CEC staff are available by <br />phone for questions and assistance, and we also offer health literacy workshops, which often take place in <br />EXHIBIT A <br />
