ARTICLE XI <br />11.0 EMPLOYEE INSURANCE <br />11.1 Health Insurance. <br />1. Effective January 1, 2018, the City shall contribute the following to insurance coverage: <br />Employee Only <br />Employee + 1 <br />Employee + 2 or more <br />$654.00 per month <br />$1,307.00 per month <br />$1,700.00 per month <br />2. Beginning Jan 1, 2020: The City shall contribute an allowance towards the employee's <br />Cafeteria Benefit plan for health insurance premiums for eligible employees and their <br />qualified dependents under The Public Employees' Medical & Hospital Care Act <br />(PEMHCA) which governs the California Public Employees' Retirement System <br />(CalPERS) Health Insurance Programs pursuant to the provisions below: <br />A. Effective January 1, 2020, the City will provide a monthly contribution to health insurance <br />premiums at the following rates: <br />• Employee Only $704.00 <br />• Employee+l $1,357.00 <br />• Employee+Family $1,750.00 <br />B. Effective January 1, 2021, the City will provide a monthly contribution to health insurance <br />premiums at the following rates: <br />• Employee Only $754.00 <br />• Employee+l $1,407.00 <br />• Employee+Family $1,800.00 <br />C. Effective January 1, 2022, the City will provide a monthly contribution to health insurance <br />premiums at the following rates: <br />• Employee Only $804.00 <br />• Employee+l $1,457.00 <br />• Employee + Family $1,850.00 <br />D.
