California Public Utilities Commission <br />Schools and Libraries E-Rate Program ("E-rate") Califomia Teleconnect Fund ("CTF") <br />Re: Letter of Authorization re E-rate and CTF Program Consulting Services <br />(hi accordance with Agreement #N-2019-187) <br />hi accordance with Agreement #N-2019-187 between the City of Santa Ana and Infinity <br />Communications and Consulting, Inc. ("Infinity"), dated July 9, 2019, the City of Santa Ana, on behalf of <br />the Santa Ana Public Library ("Library"), authorizes Infinity to prepare, certify, and submit all applications <br />and forms required by the Universal Service Administrative Co. and the California Public Utilities <br />Commission for the procurement of E-rate and CTF program discounts for Category l and 2 services to be <br />provided to the Library. These forms include FCC Forms 470, 471, and 486, and materials in support <br />thereof. <br />Infinity is also authorized to obtain account information or any other documentation deemed necessary for <br />purposes pertaining to the Library's E-rate and CTF programs, including those involving record keeping, <br />reimbursement, and services supplied by service providers, vendors, and contractors to the Library. <br />All submissions by Infinity on behalf of the Library shall be subject to review by City staff; which shall be <br />afforded a reasonable time period for such purpose. This authorization shall remain in effect through June <br />30, 2022 but may be terminated by the City at any time with or without cause upon written notice to Infinity <br />and shall automatically expire in the event the Agreement is terminated by the City.
