QUALITY CONTROL PROGRAM: <br />Proposer shall submit a Quality Control Program as part of their proposal and shall include at a <br />minimum, the following: <br />1. Monthly inspection system covering all the serviccs'listed in the Scope of Work, It shall <br />specify each site listed in the RFP mid its observed state of cleanliness, <br />2. Methods for identifying and correcting deficiencies in quality of service. <br />3. 'Proposer shall maintain a file of inspections conducted and, when applicable, tape corrective <br />action. A cagy of each inspection and documented corrective action shall be delivered to the <br />Contract Manager witinin seven (7) calendar days from the date of inspection. This collected <br />data shall become part of a general database used by the proposer as a basis of establishing <br />and modifying maintenance procedures. <br />9, QUALITY ASSURANCE <br />The Contract Manager or designee shall monitor the Proposer'$ performance using the agreed upon <br />inspeodons, standards, and schedule as outlined in this RFP. City staff shall conduct facility <br />inspections and cleaning deficiencies will be noted, A copy of the facility inspections shall be given <br />to Contractor's on -site supervisor within seven (7) calendar days of rite inspection to allow for <br />correction of any noted areas not being cleaned to the levels doseribed in the Scope of Work, If <br />continued deviation from performance standards occur, an Agreement Discrepancy Letter shall be <br />prepared by die Contract Manager within five (S) business days ookni wledging the reported problem. <br />aril presenting a program for immediate correction or presenting contrary evidence. <br />10, INVOICING <br />Invoices submitted by the Contractor must inchide the following inforxmation: <br />lnvurca l3umtr ,lnvolce Date .
