Participants will receive the support <br />HIV Care Services <br />Foundation OC <br />No. 5 <br />and resources needed to remain in medical care, maintain their health, and achieve viral suppression. <br />Brighter Futures for <br />No. 2, 2a <br />Children of prisoners deserve the chance to discover a brighter future, which is why America On Track <br />Children of Prisoners <br />America On Track <br />provides them with trained, caring mentors, STEM workshops, a "College is For Me Too!" camp, free <br />books, and special outings, as well as nutrition seminars and wraparound services for the families. <br />Boys and Girls <br />College Bound provides at -risk Santa Ana teens with academic instruction and mentors to help graduate <br />College Bound <br />Club of Santa Ana <br />No. 2, 4a <br />high school on time, prepare for secondary education, technical school, or military and set career goals; <br />this also prevents teens from engaging in activities that could otherwise derail their path. <br />Economic Empowerment— <br />Community Action <br />Economic empowerment services for low-income adults and youth in and around Santa Ana; workshops <br />Your Money, Your Goals <br />Partnership of <br />No. 3,5c <br />include Your Money Your Goals curriculum, which covers banking, spending plans, and money <br />Orange County <br />management by providing tools for participants to develop habits to increase their financial wellbeing. <br />Community Health Access <br />Community Health <br />No. 5,6d <br />CHIOC helps City of Santa Ana residents to access health and social service programs by providing <br />Program <br />Initiative Orange <br />outreach, education, enrollment, and case management services to vulnerable populations. We strive to <br />County <br />ensure that clients acquire, retain, and utilize community resources effectively. <br />Community Legal Aid SoCal (previously known as The Legal Aid Society of Orange County) will provide <br />Santa Ana Domestic <br />Community Legal <br />No. 1,61' <br />free, holistic legal assistance to Santa Ana survivors of domestic violence to help them escape abuse and <br />Violence Prevention Project <br />Aid SoCal <br />break the cycle of violence. Priority is given to service in the areas of family law, immigration, and <br />healthcare advocacy. <br />Community <br />No. 5,6e <br />The purpose of the Lunch Cafe program is to provide nutritious lunch meals for seniors in Santa Ana who <br />Lunch Cafe <br />SeniorSery <br />are isolated, low income and frail. The CDBG funds will help offset the raw food costs for providing meals <br />for senior participants living in Santa Ana. <br />Home Delivered Meals <br />Community <br />No. S,6e <br />The Home Delivered Meals program provides nutritional meals on a donation basis to seniors, who are <br />SeniorSery <br />frail, isolated, have limited mobility, and limited social support. <br />Family Economic Success <br />Delhi Center/Public <br />No. 3,5c <br />A collaborative case management strategy to help low-income families fill vocational, educational, and <br />Initiative <br />Law Center <br />financial service gaps, so they can make better decisions to earn more, keep more of what they eam, and <br />move up the economic ladder. <br />60A-12 <br />
