Comprehensiveā¢ Case Management Team meetings shall provide <br />weekly evaluations and assessment for PARTICIPANTS. <br />5.1.2.5 Termination: The assigned FRC staff, with the <br />input from the FRC Program Coordinator and/or license eligible clinician and <br />the Comprehensive Case Management Team, shall terminate the case when the <br />desired outcomes have been attained, the PARTICIPANT is non -compliant, or the <br />PARTICIPANT withdraws. <br />5.1.3 Children's Bureau shall provide Comprehensive Case <br />Management Team services continuously during the term of this Agreement. <br />Comprehensive Case Management Team meetings shall be scheduled one (1) day per <br />week for a minimum of one (1) hour in duration. The FRC Program Coordinator <br />and/or license eligible clinician shall facilitate the Comprehensive Case <br />Management Team meetings. <br />5.1.4 Children's Bureau shall provide services at the FRC and/or <br />at other community locations, to be approved in advance and in writing by <br />ADMINISTRATOR. <br />5.1.5 Children's Bureau shall measure progress by ensuring that <br />PARTICIPANTS complete a FaCT registration form, and Children's Bureau shall <br />complete a FaCT referral form, the Case Management Team Tracking and Follow -Up <br />Log, and maintain a case review log. Children's Bureau shall provide an <br />evaluation of PARTICIPANT progress using the case plan provided by the <br />Comprehensive Case Management Team. <br />5,1.6 Children's Bureau's Comprehensive Case Management Team <br />services shall address the following PSSF service categories: family <br />Preservation, Family Support, Time -Limited Family Reunification, and Adoption <br />(FNH0209) <br />18 of 42 <br />(3/18/09) <br />
