Submittal of this form, in and of itself, may not <br />provide sufficient documentation to demonstrate that goof faith effort was made. Documentation such as <br />copies of advertisement, letters of solicitation, telephone logs, rejected quotes, etc. shou ld accompany this <br />form. <br />1.
