Responsibilities of Riverside Directional Drilling Inc. (Hereinafter "RDD" or <br />"Owner"): <br />Provide directional drilling rig and all supporting equipment. <br />Provide skilled crew to operate and maintain equipment. <br />Drill, hook up pipe, pull in pipe, and handle all tools involved in this process. <br />Provide a certificate of insurance. <br />Remove any drilling slurry from site. <br />Cost of Work: <br />Directional Bore crew for placing PVC or GRC conduit(s) per plans .... $3,500.00 per day. <br />Terms: Net 30 days. Any balance due after ninety days will be charged 1.5% per month <br />(18% APR) until paid in full. Contractor to pay any court cost and attorney fees incurred <br />by RDD in order to enforce this contract. This proposal may be voided if not accepted <br />within 30 days. <br />Standard Exclusions: <br />This proposal is based upon site conditions being conducive to directional drilling. <br />Due to unforeseen soil conditions, site conditions, or existing utilities no representation is <br />made to the possible success of drilling and placing of conduit. This proposal is based on <br />soil that is free of rock, cobble, and/or trash. Additional charges may be made if these <br />conditions are encountered. RDD is not responsible for any damage to unmarked, <br />mismarked, or incorrectly potholed utilities. RDD is not responsible for any damage to <br />unmarked storm drains or sanitary sewers. <br />20 D-49 <br />
