Describe how referred clients will be linked with <br />other organizations and service providers in compliance with its use of the <br />Coordinated Entry System and to ongoing services, including physical and <br />behavioral health care and job readiness. <br />5) Admission Criteria and Procedures. Describe admission criteria and <br />procedures including client rules and guidelines, identification requirements, <br />screening requirements, bed reservation system, new clients, day leave and <br />returning clients and hours of operation. <br />6) Re -admission Procedures. Describe exit and re -admission policies and <br />procedures. <br />7) Types of Services. Describe the type of on -site and off -site services to be <br />provided including, but not limited to: housing location and navigation, outreach <br />and engagement, intake and assessment, case management, residential <br />services, and crisis intervention services. <br />8) Program Outcomes. Describe how program outcomes listed in the table will <br />be met. <br />9) Case Management. Describe the level and frequency of case management. <br />Explain how case management will increase stability. <br />IV. Service Delivery Method and Staffing — (25 points) <br />A. Provide information detailing the proposed number of staff for each program service, <br />management plan including hiring and training policies, emergency procedures, <br />conflict resolution and safety conduct. <br />B. Provide a Staff Responsibility Matrix for the HSNC including volunteer monitoring, <br />referral service and staff coordination. <br />C. Describe how the staffing plan is consistent with the Management and Operations <br />Plan. <br />D. Respondent must address the following topics in the draft Operations and <br />Management Plan (OMP). Respondent may submit and OMP for an existing <br />emergency shelter currently operating with the County of Orange. The document <br />maybe amended after award of contract for operation of the HSNC. If the OMP does <br />not include all of the required topics, additional information may be included as a <br />narrative in this section, or as a supplemental exhibit to the existing OMP. Any <br />deviations from the level of services described in the draft OMP must be clearly <br />described in this section. <br />1) Describe your Volunteer Management Plan including a plan for the <br />recruitment, training and management of volunteers for the program.
