Incentives (1 page max) <br />A. Will your organization provide incentives to youth for accomplishing program benchmarks <br />or personal goals? If so, please describe. <br />The proposed program will be utilizing incentives to acknowledge participants as they <br />reach specific program benchmarks. These benchmarks include: <br />1) obtaining employment <br />2) retaining employment <br />3) enrolling in education <br />4) retaining education (i.e. enrolling in second semester; passing semester grades) <br />5) earning an industry recognized certification <br />6) obtaining a Driver's License <br />7) completing the 6-week Work Readiness curriculum <br />8) being discharged from Probation or Parole <br />The purpose of the incentives is to encourage youth to participate in activities <br />throughout program duration, including the 12-month post -exit follow -up period. Although <br />benchmarks 6-8 are no directly related to employment and education, without achieving these <br />benchmarks, participants will often struggle to obtain and retain employment and education or <br />to sustain long-term progress in either area. They are also often a sign of readiness to change <br />and a demonstration of the participant's level of commitment to their individual success. A <br />large percentage of participants do not have a Driver's License or stable mailing address at time <br />of enrollment. By supporting participants in achieving these fundamental benchmarks, it is an <br />esteem boast as they work toward applying for jobs, enrolling in school, planning <br />transportation to and from both, and feeling confident in their personal stability and <br />professionalism to compete alongside their same age peers who may not have the same <br />struggles to contend with. <br />All incentives will be in the form of a $50 VISA gift card. Participants are eligible for and <br />encouraged to earn multiple incentives. On average, participants will earn $250 in incentives <br />throughout their participation. All incentives will be tied to training and education, work <br />readiness skills, and/or occupational skills attainment goal as identified in their ISP and/or ICP. <br />All incentives will be documented in the participant's case file and accompanied by supporting <br />documentation of the achievement. <br />B.
