EXHIBIT D <br />FEDERAL AWARD IDENTIFICATION INFORMATION <br />ATTACHEMNT D <br />FEDERAL AWARD IDENTIFICATION INFORMATION <br />The General Program Requirements were designed to provide the framework where the <br />CONTRACTOR will provide CARES Act Coronavirus Relief Programs identified in this <br />attachment. <br />I. <br />The CONTRACTOR agrees to comply, remain informed, and deliver services consistent <br />with the provisions of the Coronavirus Aid, Relief, and Economic Security Act ("CARES <br />Act") Section 5001 under the Social Security Act Section 601(a). <br />Where local policy has not been set, The CONTRACTOR agrees to adhere to state and/or <br />federal policy, as appropriate. <br />II. GOVERNANCE REFERENCES <br />A. Additional state and federal agencies that provide funding to the CITY may be <br />incorporated herein. <br />B. Information Bulletins, Directives, and any other federal and state guidance documents <br />pertaining to the CARES Act Coronavirus Relief Funds. <br />C. Actions, directives, and policy and procedures issued by the CITY. <br />D. CITY policies, as applicable. <br />III. CONTRACTOR/SUBRECIPIENT DETERMINATION: <br />In accordance with the requirements of 2 CFR 200.330 (Subrecipient and CONTRACTOR <br />determination) and for the purpose of this CONTRACT, Managed Care Solutions Services <br />Corporation is determined to be a CONTRACTOR. <br />26 <br />
