A. Statement of Qualifications <br />In order to maintain uniformity with each Consultant, the Statement of Qualifications <br />must be limited to a maximum of 20 pages (excluding front and back covers, table of <br />contents, section dividers and resumes). The page limitation includes all appendixes, <br />attachments and supplemental information. Except as specified above, each page of <br />text will be counted individually i.e. pages printed on the front and back will count as <br />two pages. The following information is required: <br />1. Cover Letter <br />A letter signed by a principal or authorized officer who may make legally binding <br />commitments for the entity. The scope of services that the firm is qualified for should <br />be identified within the cover letter along with the primary project contact and contact <br />information. <br />a. Scope of Services <br />Clearly identify the scope of services that will be provided (if one, two or all <br />three services can be provided). Services to be provided should be identified <br />by both the letter and name that is listed in Section III above (i.e. "A. <br />Environmental Services," "B. Technical Studies — Historical Assessments," <br />"B. Technical Studies — Air Quality, Noise and Traffic, etc.). <br />b. Project Contact <br />The Project Manager/Principal Agent for the proposed work shall be <br />identified. The contact's name, email and phone number should be included. <br />The project manager shall be the primary contact person to represent your <br />firm and will be the person to conduct the presentation, if invited for an <br />interview. <br />2. Firm and Personnel Experience <br />A profile of the firm's experience in producing environmental documents and <br />technical studies and a brief resume of key personnel who may perform work for the <br />City. <br />3. Relevant/Related Experience <br />Provide a list of projects, which your firm or personnel have completed within the <br />last 5 years, including significant work with public agencies. Project information <br />should include project description, year completed, client name, along with a person <br />to contact and their telephone number or email. <br />5 <br />19E-8 <br />
