3. ADDITIONAL REQUIREMENTS <br />REQUIREMENTS <br />YES <br />NO <br />COMMENTS AND EXCEPTIONS <br />a) Contractor, Contractor's Employees and <br />Contractor's Subcontractors shall wear <br />uniforms with company name and <br />employee nametag visible and company <br />vehicle must be properly marked with <br />company name. <br />b) Contractor, Contractor's Employees and <br />Contractor's Subcontractors shall <br />Perform all work in accordance to all <br />City, State, and Federal Safety <br />Standards and Regulations. Contractor <br />shall obtain a copy of the City's Safety <br />Policies and Procedures form the City's <br />Risk Management Office. _ <br />c) Upon entering the job site, Contractor, <br />Contractor's employees and the <br />Contractor's Subcontractors shall check in <br />with a City employee (in person or by <br />(phone) and document start time on work <br />order. Upon job completion, Contractor <br />shall document time completion and have <br />work order signed by a City Employee. <br />d) The City designee will identify the type <br />of service response required when the <br />request is initiated. <br />d1) EMERGENCY RESPONSE: <br />Phone response within 30 minutes of <br />the initial call.
