Work Order #1 24/7 COVID Nurse Line Services The services provided under this Work Order #1 are provided pursuant to the Services Agreement between Health Dialog Services Corporation ("Health Dialog") and the City of Santa Ana ("Client"), with an Effective Date of September 15, 2020 (the "Agreement"). Capitalized terms used in this Work Order #1 shall have the meaning provided herein, or if not defined herein, shall have the same meaning as in the Agreement. This Work Order #1 describes the Health Dialog Services to be provided to W-1 Eligible Members (as defined below) pursuant to this Work Order #1 (the "Nurse Line"). The start date for the Nurse Line shall be September 28, 2020 (the "NL Effective Date"). Health Dialog shall provide the Health Dialog Services described below, as refined during the implementation planning activities and updated as agreed between Client and Health Dialog. The Health Dialog Services will be provided to W-1 Eligible Members, as defined below, as a single population in a standardized structure and format. This standardized structure includes, but is not limited to, the following components of the Nurse Line services: optional general awareness mailer; Health Coach activities; reporting; implementation activities; and data feed timing and formats, all as described further below. Customization of Health Dialog Services will be considered by Health Dialog at the request of Client. Health Dialog will work to accommodate such customization requests to the extent reasonably possible and to the extent reasonably possible within the timeframes available to meet targeted program launch dates. Such customization shall be subject to the prior written agreement of the Parties including, without limitation, any adjustment to the fees (as defined in Section VI I, below) with respect to such customized Health Dialog Services. I. W-1 Eligible Members Under this Work Order #1, Health Dialog will provide the Health Dialog Services described herein for individuals who are identified by Client as residing in eligible Santa Ana ZIP codes (set forth in Attachment A to this Work Order #1) ("W-1 Eligible Members"). On the NL Effective Date, there will be approximately 350,000 W-1 Eligible Members. Outreach Services At Client's option, Health Dialog will provide a general awareness mailer ("GA Mailer") sent to W-1 Eligible Member households for the additional fee listed in Section VII.A below. Client must provide a list of W-1 Eligible Member households for the mailing, in a format to be agreed upon between Client and Health Dialog.
