IV. <br />NOTICES <br />The SUBRECIPIENT and the CITY agree that all notices required by this AGREEMENT shall be <br />made in writing and delivered via mail (postage prepaid); commercial courier; personal delivery; <br />or sent by facsimile or other electronic means (provided that receipt is confirmed). Any notice <br />delivered or sent as aforesaid shall be effective on the date of delivery or sending. Box 1905 <br />Santa Ana, CA 92702 <br />Attn: Larry Haynes, Executive Director <br />The SUBRECIPIENT must work with the Continuum of Care ("CoC") to ensure the screening, <br />assessment, and referral of program participants are consistent with the CITY's written standards <br />for providing ESG-CV assistance as described in its consolidated plan. The SUBRECIPIENT <br />must keep documentation evidencing the use of, and written intake procedures for, the centralized <br />or coordinated assessment system(s) developed by the CoC in accordance with the requirements <br />established by HUD. See 24 CFR 576.400. <br />B. Evaluation of Program Participants Eligibility and Needs <br />The SUBRECIPIENT must conduct evaluations and re-evaluations to determine the eligibility of <br />each individual or family's eligibility for ESG-CV assistance in accordance with 24 CFR <br />576.401. <br />
