June 30, 2021) <br />Name of Organization City of Santa Ana Parks Recreation and Community Services Agency <br />Name of Funded Program Cesar Chavez Campesino Park Restroom <br />Annual Accomplishment Goal <br />Program and Funding Description <br />Project location: The Cesar Chavez Campesino Park Restroom Replacement Project ("proposed <br />project' or "project") Is located at 3311 West 5th StTeet In the City of Santa Ana (City) in Orange <br />County, California 92703. Figure 1 illustrates the general location of the project site in the region <br />and Figme 2 shows the location of the project site in the community and surrounding uses. Figure 3 <br />Includes a site plan, which depicts the general location of the existing restroom facility within the <br />approximately 6.3-acre neighborhood park. See Appendix A for Figmes 1, 2, and 3. <br />Description of the Proposed Project (24 CPR 50.12 & 58.32; 40 CFR 1508.25j: <br />The proposed project involves demolition and replacement of an existing restroom building within <br />Cesar Chavez Campesino Park. The new, prefabricated , and ADA•compliant restroom facility would <br />remain in the same location as the existing restroom building , <br />EXHIBIT A <br />
