Assurances & Certifications <br />Selected providers will be required to sign and submit "actual" assurances and certificates as required by <br />the City of Santa Ana and the Workforce Development Board on all contracts. <br />I recognize that I must give assurances for each item below. Please initial each box indicating you have <br />read and are providing assurance you are or will be in compliance with the following: <br />14 SAWDB uses the Ca1JOBs ETPL Module for accepting applications from the providers to be listed <br />on the ETPL. Local boards may authorize a single local board to act on their behalf in making <br />determinations for initial and/or subsequent eligibility of providers. Contractors must enter program(s) <br />of training services into CaIJOBS. The training provider should only enter the program(s) desired to <br />be on the CA ETPL. If the program is offered with multiple modes of delivery, or course lengths, the <br />program must be entered separately for each variation. <br />10 SAWDB has authorized the South Bay WDB to make initial and subsequent eligibility determinations <br />for applications submitted from providers. Contractors should contact the South Bay WDB 11539 <br />Hawthorne Blvd., 51 Floor, Hawthorne, CA 90250, 310-970-7700. <br />CONTRACTOR is required to be in compliance with South Bay WDB Master agreement and <br />SAWDB agreement. <br />❑� In cases where South Bay WDB has denied a provider's application, provider may submit the <br />application to SAWDB for consideration and processing at 801 W. Civic Center Dr. 4200, Santa Ana, <br />CA 92701 ATTN: SAWDB Staff. The submission must include completed ETPL training program <br />applications and a copy of the letter from the South Bay WDB denying application. <br />aLocal boards may establish local policies requiring performance above the state minimum standards <br />for providers to be included on the ETPL. <br />ZAcceptance and processing of an application does not constitute an agreement or relationship between <br />the CONTRACTOR and SAWDB, nor does it guarantee any referrals to the provider by SAWDB. <br />Applications will be process on an ongoing basis. <br />ZCONTRACTOR must be in compliance with the State and Federal regulations, per Workforce <br />Innovation and Opportunity Act (WIOA) Eligible CONTRACTOR List Policy and Procedure <br />WSD15-07 or it's replacement. <br />ZCONTRACTORS must also meet one of the following criteria in order to have their programs <br />listed on the ETPL: <br />1) Bureau of Private Postsecondary Education Approval to Operate, or Verification of <br />Exemption by BPPE. <br />EXHIBIT G <br />
