Contractor <br />You are hereby directed to make the raisin changes from the plans and specifications or do the following descdbed work not included in the plans and <br />specifications on this contract. <br />NOTE: THIS CHANGE ORDER IS NOT EFFECTIVE UNTIL APPROVED BY THE CITY COUNCIL OR CITY MANAGER. <br />Unless otherwise stated, rates for rental of equipment cover only such Ome as equipment Is actually used and no allowance will be made for idle time. <br />Change requested by PUBLIC WORKS AGENCY <br />EXTRA WORK AT AGREED PRICE <br />COP No. 03 R1 10th STREET EXTRA WOR <br />Cost associated with T&M work done on 101h Street. <br />In accordance too waterline design plans and specifications, Excel Paving marked, saw cut paving <br />and exposed existing utilities (marked & unmarked) throughout 101h and Bush St. Existing utility <br />elevations and alignments are in direct conflict with the new mainline design. The City provided <br />revised plans to Excel paving for a Cost proposal. Excel Paving provided a lump surn price of <br />$54,000.00 to complete the work in accordance to the revised plans. New issues surfaced, causing <br />additional plan revisions. Therefore, in order to keep the project moving, the City provided approval <br />for the contractor to proceed with the work on a time and material basis.
