Applications reviewed by Latino Health Access will be those that have not received prior funding through CARES for <br />Tenants and will be eligible for $5,500 in rental assistance funding. <br />-Latino Health Access will administer $2,408,675.95 in direct financial assistance for approximately 438 families by providing <br />a one-time payment of $5,500. If not all 438 families are eligible for $5,500, Latino Health Access will assist additional <br />families up to the maximum amount of$2,408,675.95 in direct financial assistance by providing a one-time payment of up to <br />$5,500 per family. <br />-Direct assistance may only be approved in compliance with Section 501 of Division N of the Consolidated Appropriations Act <br />of 2021, the Department of Treasury's Frequently Asked Questions, the CARES for Tenants Program Guidelines and <br />Frequently Asked Questions established for the Program. <br />Eligibility Criteria: <br />-Santa Ana resident with an obligation to pay rent <br />-Household income below 80%of the area median income (with a priority for residents with income below 50%the area <br />median income) <br />-A household memberthat has a reduction of income or otherfinancial hardship due to COVID-19 <br />-At risk of experiencing homelessness or housing instability <br />Program Details: <br />-Residents will apply online via the Neighborly Software application available on the City of Santa's website following the <br />CARES for Tenants Program Guidelines and Frequently Asked Questions. <br />-Latino Health Access will determine the eligibility of tenants for the program, including: <br />application review, eligibility verification, collection of required supporting documentation, communication/coordination <br />with the tenant, coordination with the tenant's landlord for the tenant's lease/sublease agreement and rent amount, and <br />EXHIBIT A <br />
