respond in writing to an appeal within one week of a decision by the Committee. If a student's appeal is approved, he/she <br />will be placed on Financial Aid Probation and will be provided an SAP academic plan. While on Financial Aid Probation, the <br />student is eligible to receive financial aid for ONE quarter. In order to be eligible for aid for subsequent quarters, the <br />student must meet the terms of the SAP academic plan. For all students on Financial Aid Probation this includes: <br />Satisfactorily complete all courses attempted during the quarter. <br />Maintain a minimum quarter GPA of 2.0 (undergraduate). <br />Additional requirements may be listed on the appeal approval notice. Failure to meet the requirements specified by the SAP <br />academic plan will result in losing financial aid eligibility for all future quarters until all satisfactory academic progress <br />components are met. To re-establish financial aid eligibility, all academic deficiencies must be eliminated by completing the <br />required number of credit hours necessary to meet 2/3 (66.67%) pace and/or complete enough credit hours to achieve the <br />minimum 2.0 cumulative GPA (undergraduate) at the student's expense. <br />Per federal regulation, if a student fails an academic plan, he or she may not be granted an additional plan. In cases <br />where a new, unexpected extenuating and documented circumstance exists, a new SAP appeal may be considered. <br />The extenuating circumstance in the new appeal cannot be the same as the circumstance outlined in the previous <br />appeal. The new circumstance must also have occurred within the quarter for which the academic plan was failed. <br />Even in this latter case, an additional approval to receive financial aid is not guaranteed. <br />All information is subject to change based on changes to federal law, regulation, or college policy and procedure. If changes <br />are made, students must abide by the new policy. Note: Due to changes in federal regulations, the described Financial Aid <br />Satisfactory Academic Progress Policy became effective the 2018-2019 award year. <br />ACADEMIC DISMISSAL <br />Students may be dismissed for academic reasons and will be notified in writing of dismissal. Students who are dismissed are <br />required to return all materials loaned to them by the school. Serious infractions such as intoxicated behavior, cheating, <br />threatening, or violent behavior are cause for immediate dismissal. Students seeking to re-enter after a voluntary withdrawal <br />must follow the re-entry policy. <br />ACADEMIC INTEGRITY POLICY <br />All students are expected to make choices that reflect integrity and responsible behavior. It is the instructor's responsibility <br />to report academic dishonesty to the appropriate Designated Official. Violations of the Academic Integrity Policy include but <br />are not limited to: <br />Copying work, ideas, or projects from any other person or media, copying answers on a quiz or test, or communicating <br />with another person during a quiz or test <br />•S Allowing another person to copy or borrow original work in any form <br />❖ Claiming ownership of the work of a team member <br />❖ Committing plagiarism in any form, including failing to properly cite the source of thoughts, words, ideas, or work from <br />any other person, printed material, or website <br />t• Committing "self -plagiarism" by submitting work previously submitted in another course unless it is properly cited <br />When a violation of this policy occurs, disciplinary action will be taken. Based on the severity of the violation, academic <br />dishonesty may result in an "F" grade for an assignment, project, assessment, or the course itself. Multiple violations may <br />result in dismissal from the college. <br />LEAVE OF ABSENCE <br />If an emergency arises, such as a family tragedy, medical condition, or military obligation, malting it necessary for a student <br />to interrupt his/her training, the school, at its discretion, may permit a student to request a Leave of Absence (LOA). Due to <br />the scheduling of the Vocational Nursing program and Phlebotomy course, students are not allowed to take a Leave of <br />Absence*. The following criteria are required: <br />d• The student must request the leave in writing (prior to the expected LOA) and must sign and date a request which states <br />a reason. <br />❖ The applicable School Official must approve and sign the LOA request prior to the start date of the LOA. <br />❖ The reason for the request must provide reasonable assurance that the student will return at the end of the LOA. <br />8• The school may ask for documentation confirming the reason for the LOA. <br />Acceptable reasons for LOAs include but are not limited to: medical treatment, death in the immediate family and <br />military obligations, etc. A LOA may not be granted for reasons associated with a student's academic performance. <br />-e A LOA will not be granted during the first didactic module unless the student requests a reasonable accommodation <br />under the American's with Disabilities Act (ADA). <br />26 <br />
