Description of Work - In the space below, describe the program to be funded during the 12-month contract period. <br />What specific activities will be undertaken during the contract period. Only the <br />viewable space will print. <br />Our core transitional program, Steps to Independence provides short -to -medium -term shelter and wraparound <br />services for up to six months for unaccompanied homeless women including mandatory financial <br />empowerment curriculum. WISEPIace also provides a second step program for chronically homeless women <br />and a rapid rehousing program. Our Rapid Rehousing complements the Steps to Independence program by <br />applying proven prevention techniques and a focus on shortening the length of time a woman remains in a <br />homeless state. <br />Both our Steps to Independence program and Rapid Rehousing services include preparing each woman to save <br />for her future and educating them on being a good tenant to help ensure they are self-sufficient and stay <br />housed. Staff work with each woman to create goals and a personal housing plan. Women also participate in <br />life skills classes which include resume writing and job readiness. <br />Our Rapid Rehousing, Housing Relocation and Stabilitization Services include assistance with rental application <br />fees, security deposits, and/or services such as case management, housing search and placement, legal <br />services. etc. Part of WISEPlace's exit interview process is to confirm the location where the client is moving. A <br />request for follow up home visits are made within three, six and twelve-month periods. <br />We anticipate serving 15 unduplicated Santa Ana woman during the 12-month period with ESG funding. <br />Schedule of Performance <br />Estimate the number of unduplicated Santa Ana residents to be served by the funded program during the 12-month <br />contract period per quarter.
